Employee Referral Form

In order to begin the referral process, you must read and complete the following form. After reading
the policy, please complete the form and submit it with the applicant’s resume to your Human
Resources team (hr@simplexgroup.net).

REFERRAL GUIDELINES

• You are eligible for a referral bonus only when you refer external candidates. The referral must
represent the candidate’s initial contact with Simplex. If a candidate has previously applied on-
line or otherwise, no referral bonus will be paid.
• If the candidate you refer is hired, you will receive a referral bonus of $225 (Tier1) or $100 (Tier2)
after the new employee has worked at Simplex Group for two months.
• The Employee Referral bonus is not applicable:
1. For a candidate who is being rehired
2. For a candidate referred in the past 60 days or more
3. For a candidate already working for another Simplex Group partner
4. If a personnel agency or employment search firm has already been paid for the referral, or is
owed a fee for the referral
5. For someone who is a temporary employee, freelancer or consultant
6. If you are:
a. An employee who has a direct reporting relationship with the position being filled
b. A relative of the employee(s) hired
c. An employee working in any Human Resources capacity
d. An employee whose responsibilities include recruiting

• Only one referral bonus can be given per candidate. If a candidate is referred by more than one
employee, the first referral received will be the one rewarded if the candidate is hired.
