Agenda 1: Opening of the meeting

The chairperson opened the meeting at 03:30 PM and explained in brief on the
consultancy service needed to be provided by the consultant in terms of his
qualifications, experience and quotation costs for the project (Integrated Livelihood for
building resilience project in Mbulu. The Chairperson insisted that the consultancy
assignment needs to be done as soon as possible so as to be completed before the end
of financial year that is October, 2021.

Agenda 2: Selection of consultant to conduct evaluation of the project

According to the report submitted by the evaluation committee, three applications for
consultancy services were received and opened as follows:

Having reviewed and discussed critically the report submitted by the evaluation
committee and based on the terms of reference, the Consultant Selection Committee
Meeting approved Mr:Salutary S. Umbu to conduct the evaluation of the named project.

As reported by the evaluation committee, among other things, the following reasons were
also considered to approve Mr. Salutary S. Umbu.

1. He is ready to conduct the evaluation work within the planed time.

2. He has required qualification and experience in project evaluation

3. He is in Babati hence easy to communicate in case of anything

4. His price is fare.

Agenda 3: Closing the meeting

The meeting was declared closed by the chairman by thanking the member for their full
and in-depth participations.
