This document provides a cable test data form and instructions for testing cables.
The form collects information about the cable such as the project, circuit description, date, cable size and length. It also records test data including the applied DC test voltage over time and the resulting current in microamperes on each of the cable's phases.
Instructions specify that the initial DC test voltage should be no more than 3 times the rated AC voltage and that it should increase uniformly over 10 to 60 seconds to the specified test voltage. Shield cables are tested at the voltage for 15 minutes and non-shield cables for 5 minutes. A graph is included to plot the current over time for each phase.

The rate of increase from the initially applied voltage to the specified test voltage shall be approximately uniform and shall be not over 100 percent in 10 seconds nor less than 100 per cent in 60 seconds.
