A. PRELIMINARIES
A.1 Insurance
Provide necessary insurances required under contract:-
a) Contractor All Risk
b) Third Party Liability
c) Workmen Compensation
(Insured Period : Construction period + 1 year defect. Liability period + 6 months)
A.2 Local Authorities Submission And Approval

To provide, submit and seek for Government Authority approval:-
i. DOE
ii. DOSH
iii. BOMBA (notification confined space work)
(Copy of submission and approval letter must submit to Engineer)
A.3 Temporary Site Office & Buildings

Provide, maintain and remove Portable Office Cabin for site office usage, document
storage and management.
A.4 Worker Accomodation & Storage

Establish worker's living quarters, stores, site facilities and removal upon completion
work. This include survey and first aid outfit.
A.5 Mobilization And Demobilization Transport

Allow for mobilization and demobilization of transport to and from site to all plant,
equipments and labour necessary for the execution of the works including all duties and
insurances, etc. and providing for the temporary crossing and drainage necessary for
proper execution of works.
A.6 Safety & Health

Provide safety precautions to ensure the health and safety of Contractor's staff and
labour. This include providing personal protection equipment (PPE), medical kit,
portable fire extinguisher etc. based on HIRARC assessment.
A.7 Site Safety and Health Supervisor

The employment of competent Site Safety and Health Supervisor for the entire duration
of the construction period to be stationed at site. This include report to summarize
occurences, accidents and safety performance.
A.8 Authoruzed Gas Tester Personel (AGT)

The employment of competent authorized gas tester to ensure tank in safe condition to
work. This include carry out air testing for toxic, flammable gas and fumes and oxygen
adequacy before access to the confined area of work.
A.9 Leak Test

Provide test to verify welding condition and check leakage for each unit tank.
