AFFIDAVIT FOR NO 3 (THREE) WORK IN HAND

1. I the undersigned do hereby swear that all the statement made in the required
attachment is true and correct. In case of the documents pertaining to tender submitted by
me are found to be incorrect or false, my firm may be rejected at any stage and I may be
liable for action under provision of Indian Penal code and other relevant law.

2. The undersigned further do greedy certify the my firm M/S T.D Enterprise is not engaged
in more than any 3 (three) tender work under state Govt./PSU/Corporation till date as per

3. The undersigned also do hereby certifies that neither my firm M/S T.D Enterprise have
abandoned any works on APGJWCL/PWD/ and RWD etc. nor any contract awarded to me
for such works have been rescinded during last five years prior to the date if bids.

4. The undersigned hereby authorized and request any Bank person, firm or corporation to
furnished pertinent information deemed necessary and request by department to verify this
statement or regarding my (our) competence and general reputation.

5. That undersigned understand and agrees that qualifying information may be requested
and agrees to furnish any such information at the request of the department/project
implementing agency.

6. I Shri Tara Daniel Proprietor/legal representative of M/S T, Enterprise do hereby declare

that I shall rectify the defects occurred in the structure for a period of 3 (three) years from
the date of completion of the works. Further I may also be held liable for structure failures,
if any caused due to sub standard works done by my firm.
