You are required to carry out the requisite tasks after carefully reading through the scenario.

Scenario:

You have been appointed as safety advisor in a construction company having operations in a site. You are asked to
cover all health and safety related matters. You are told that more than 100 employees are already working the site
and more are expected to be hired as the operations move on. You are further informed that a safety officer was
already hired but he left for his personal reasons and did not want to carry on his duties. CEO of the organization
welcomes you and asks you carry out your duties with responsibility to ensure organization is able to fulfill its legal
duties. You are then directed to proceed to the main project, located in a main city. When you reach the site, you
observe that site is located by a busy road. Site vehicles are parked outside and blocking the routine traffic. There I no
signage or arrangements for handling traffic and other road issues which may be faced. The entrance has no sign board
or access control. There is a person sitting beside the gate who tells you he is a security guard in this company and
works for twelve hours. On further query he informs sometimes he has to perform more than twelve hour’s duty so
he have been provided with a cabin beside the gate. You approach project manager’s office to report your arrival.
While discussing with the project manager you are informed only a mission statement is available to guide you for
health and safety related affairs. The project manager brings a copy of the H&S statement of intent copy from his
drawer and hand it over to you. He then asks an employee to guide you to the supposed sitting arrangement for you.
You see a lot of poor housekeeping on the way. Materials are scattered everywhere and no persons are engaged in
housekeeping activity. You just saw a person just tripped over the materials and escaped a fall. The accompanying
employee remarked this is a daily routine of the site and nobody reports such incidents. He further tells nobody listens
to our reports and if someone reports again then the reporting employee has to face problems from the colleagues as
well as from managers and supervisors. When you reach at your assigned office, you observe that there is no record
of any health and safety related matters. You open up the desktop computer to check out whether some records are
maintained in this desktop. It is a pleasant surprise to find data regarding:

4. Permit to work for different activities
5. Safe system of work for various dangerous activities

You now decide to visit the site to observe the health and safety related conditions prevailing at site. You observe hot
work being carried out without permit to work, no toolbox talks being carried out for any activity. You even don’t see
safety signs or a copy of safety policy displayed. When you ask one supervisor, he laughs and responds in a queer way:
We have never seen such things in our site and our seniors think it a waste of time. He further told you that there is
no formal and informal training provided to enhance competence of workers or their seniors. You observe that no
edge protection is provided at the second and third floor of the building. Workers moving around the edges unsafely
and some observed tripping on materials placed on the slab. You further observe Filthy toilets, a limited supply of
clean drinking water, no rest rooms, no change rooms, and only one first aid kit for more than 100 employees.

After carrying the inspection you decide to talk to your project manager about these poor health and safety related
conditions.

Task 1: As a safety advisor you are clear that company has failed to fulfill their legal duty regarding health and safety.
