1. Purpose and Scope

1.1 General
This Fall Prevention Plan establishes the guidelines that will be implemented to
manage construction site activities which are mainly involved with working at height
operation at project during the execution of (project name) located at (address)

This Fall Prevention Plan shall be in line with the principles of relevant Statutory
Regulations, Client working at height procedure requirements and Rotary HSE
management system requirements.

1.3 Project Scope


This document explains Rotary Electrical & Instrumentation Pte Ltd (REI) Fall
Prevention Program implementation at site construction activities that is to be
applied to (Project Name) under (client) Electrical Scope of Works for E&I Package.

1.4 Documentation Distribution & Control


This Fall Prevention Plan is reviewed, accepted and signed by Project Manager/
Director and shall be updated as and when required. The document is distributed to
the following project members / functions:

1.5 Confidentiality
REI will fully adhere to the confidentiality requirements. The Project Manager will be
responsible to ensure that these requirements are satisfactorily communicated
within the project, and to all sub-contractors.

1.6 Limitations and Assumptions


This Fall Prevention Plan is applicable only throughout the execution of AP2WML +
PRISM Project.

2. Standards & References

REI shall maintain both overall & specific Safety requirements, which requires the
Project to comply with:
 REL-HSE-08-05 (Working at Height Procedure)
 REL-HSE-10 (In-house Rules & Regulations Procedure)
 REL-HSE-11 (HSE Promotion Procedure)
 ISO 9001:2008 (Quality Management System)
 ISO 14001:2004 (Environmental Management System)
 OHSAS 18001:2007 (Occupation Health and Safety Management System)

3. Electrical Scope of Works – For E&I Package Policy for Fall Protection

REI is dedicated to the protection of its employees from occupational injuries associated
with working at height, in compliance with Singapore legal legislations & standards. The
purpose of this plan is to supplement REI Health, Safety & Environmental Management
System and to ensure that every employee who works for REI recognizes workplace fall
hazards and take reasonably practicable measures to address those hazards.

4. Responsibilities

4.1 Employer
REI shall provide fall protection to affected employees, and ensured that all
employees understand and adhere to the procedures of this plan and follow the
instructions of the Competent Person(s).

4.2 Competent Person(s)


The Competent Person(s) shall be appointed to develop, implement, maintain and
evaluate the fall protection plan. The fall protection plan shall be developed specific
to the conditions of the project.

It’s the duty of the Competent Person(s) to implement this plan by:

4.2.1. Performing routine safety checks of work operations;

4.2.2. Enforcing REI safety policy and procedures;

4.2.3. Correcting any unsafe acts or conditions immediately;

4.2.4. Training employees in recognizing fall hazards and the use of fall
protection system;

4.2.5. Maintaining records of employees training, equipment issue, and fall


protection systems used in project;

4.2.6. Investigating and documenting all incidents that result in employee injury.

4.3 Designated Qualified Person(s)

The Designated Qualified Person(s) shall be appointed by the project management


team in their respective subject fields.

It’s the duty of all designated qualified person(s) to:

4.3.1. Maintain professional certification or other requirements in their subject


field;
4.3.2. Provide design, analysis, evaluation and specification in their subject field;
4.3.3. Maintain records of their designs, analyses, evaluations and specifications
according to the requirements of the Fall Protection Plan.

4.4 Supervisors

It’s the duty of all supervisors to:

4.4.1. Ensure that employees are informed, trained, and provided with the
appropriate fall protection systems and equipment to be protected from
potential fall hazards associated with their job tasks;
4.4.2. Coordinate the correction of fall hazards brought to their attention by
employees; and
4.4.3. Produce any additional documentation needed to investigate any work
related injuries or illnesses.

4.5. Employees

It’s the duty of all employees to:

4.5.1. Understand and adhere to the procedures outlined in the Fall Protection
Plan;
4.5.2. Follow the instructions of the Competent Person(s);
4.5.3. Bring to management’s attention any unsafe or hazardous conditions or
practices that may cause injury to either themselves or any other
employees;
4.5.4. Report any incident.

5. Risk Management

Prior to starting any work, risk management shall be carried out in accordance to REL-
HSE-01 (HSE Risk & Impact). It must be noted that while there are several identified
tasks where fall protection is normally required, every task undertaken by a worker can
have inherent risks associated with it.

A fall hazard exists when:

a) A worker will be within 2 metres of an unprotected edge; and/or


b) May fall 1.8 metres or more, or
c) There is an unusual possibility of injury if the worker falls less than 1.8 metres
(for the purpose of this Fall Protection Plan, there is an unusual possibility of
injury if the injury may be worse than an injury from landing on a solid, flat
surface).

When a work at height hazard is identified, a work procedure shall be developed to


ensure that the risk of working at height is minimized, or if at all possible eliminated.

6. Risk Control Measures

In accordance to REL-HSE-08 (Operational Control), REL-HSE-10 (In-house HSE Rules &


Regulations) and REL-HSE-01 (HSE Risk & Impact), as far as reasonably practicable, the
appropriate control measures shall be implemented to reduce or maintain the risks of
working at height.

The control measures may consist of but not limited to the following:

6.1. Access and Egress


6.1.1. Means of access and egress with secure handhold and foothold shall be
provided
6.1.2. Adequate natural or artificial lighting shall be provided to all access ways
6.1.3. All access ways shall be cleared of obstruction
6.1.4. Access ways shall be inspected regularly especially after exposure to
adverse weather
6.1.5. Designated area for material storage via prior planning

6.2. Controlled Access Zones


6.2.1. Only authorized employees are permitted to enter controlled access zones
and areas from which guardrails have been removed
6.2.2. All other personnel are prohibited from entering
6.2.3. Controlled access zones shall be defined by control lines consisting of
ropes, wires, tapes or equivalent material, with supporting strong vertical
poles, and shall be:
 Flagged with a high visibility material
 Rigged and supported from the walking / working surface
 Strong enough to withstand the impact of a person falling against it
 Extended along the entire length of an unprotected or leading edge
 Parallel to the unprotected or leading edge

6.3. Guarding of Openings


6.3.1. Barriers should be erected around openings to prevent persons from
falling through
6.3.2. Openings shall be covered with material of adequate strength to prevent
entry of object or persons
6.3.3. Suitable signage shall be placed nearby or onto the protective cover itself
to warn people of the hazard

6.4. Edge Protection / Guard Rail


6.4.1. Guard rails must be provided at unprotected edges or wherever a person
is at risk of falling from open sides
6.4.2. Toprails must be at least 1m above the working surface with midrails and
toe-boards provided
6.4.3. Guard rails shall be designed and constructed to withstand the impact of a
person falling against it

6.5. Excavations
6.5.1. Fall protection shall be provided to employees working at the edge of an
excavation that is 1.5 metres or deeper
6.5.2. Excavations that is 1.5 metres or deeper shall be provided with edge
protection / guard rail, barricades or covers
6.5.3. Walkways that allow employees to cross over an excavation that is 1.5
metres or deeper shall be equipped with guard rails

6.6. Work on Roof


6.6.1. Brittle or fragile areas shall be identified and communicated to affected
person prior to commencement of roof work
6.6.2. Anchorage provided for the attachment of safety harness shall be
sufficient and secure
6.6.3. Safe access and working platform shall be provided to ensure work to be
performed safety
6.6.4. Only trained & competent person shall perform work on roof
6.6.5. Danger signage shall be displayed at all points of access to the roof

6.7. Ladders and Step Platforms


6.7.1. Ladders shall be properly secured against firm surfaces and/or support by
co-workers
6.7.2. Ladder base shall be located approximately a quarter of the vertical height
of ladder as shown in the figure below:

6.7.3. Where ladder is used as a means of access or as a working place,


adequate handholds should be provide to a height of at least 1m above
the place of landing of the highest rung to be reached by the feet of any
person working on the ladder
6.7.4. Three points of contact shall be maintained on the ladder at all times
6.7.5. Materials and tools shall not be carried by hand when ascending or
descending the ladder. Tool belt shall be worn if necessary. Lifting bag
can also be used to hoist items to and from height, thus freeing the hands
6.7.6. Slip resistant footwear shall be worn when using the ladder
6.7.7. Ladders shall be inspected before and after every use
6.7.8. Damaged or defective ladder (such as missing or defective runs,
deformed shape, etc.) are not permitted to be used

6.8. Scaffolds

6.8.1. All scaffolds and its components must meet the requirements stipulated in
the Workplace Safety & Health (Scaffolds) Regulations 2011. Basic
components of a scaffold are shown below:

6.8.2. Structures and foundation scaffolds shall be of good construction, sound


materials, and of adequate strength for its intended load
6.8.3. Scaffolds exceeding 4 metres and tower scaffolds shall be erected,
installed, altered or dismantled by trained and competent scaffold erector
6.8.4. Each bay shall not exceed permissible maximum load of tools, materials
and number of persons
6.8.5. Adjustments, alterations or unauthorised operations shall not be practiced
after initial installation, except for performing regular checks or
maintenance
6.8.6. Inspection and maintenance shall be carried out regularly by competent
person depending on working conditions and not less than once a week

6.9. Mobile Elevated Work Platforms (MEWPs)


6.9.1. MEWP shall only be used on solid and even ground surface within the
equipment load limit
6.9.2. Personal fall protection equipment (i.e. harness) shall be worn and
anchored securely to the guardrail of the MEWP
6.9.3. Operator shall be trained and certified competent before operating the
MEWP

7. Personal Fall Protection Equipment

Personal Fall Protection Equipment shall be issued to and used by employees as


determined by Competent Person and may consist of anchorage, connectors, body
harness, deceleration device, lifeline, or suitable combinations.

Personal Fall Protection Equipment should only be used as the last resort, after all other
control measures have been considered, or as a short term contingency, or as an
additional protective measure.

The effectiveness of personal fall protection equipment as a control measure is


dependent on the correct equipment being chosen, fitted and worn properly at all times
when required by users. As such, the following factors must be taken into consideration:

 Type of hazard;
 Area of the body that require protection;
 The degree of protection required;
 Ease of use, comfort and convenience;
 Reliability; and
 Ease of maintenance.

Personal Fall Protection Equipment is divided into 5 functional categories:

i. Fall Arrest
The use of a personal fall arrest system is the required personal protective
equipment for fall hazards for AP2WML + PRISM Project. A personal fall arrest
system consists of a full-body harness, lanyard, and anchor point or other
suitable combinations. All fall protection equipment shall meet or exceed
appropriate American National Standards Institute (ANSI) standards. Project
Name shall only use commercially manufactured equipment specifically designed

for fall protection and which are PSB certified. All fall protection equipment must
bear the marking of the manufacturer and approvals for specified use.
Requirements for a personal fall arrest system include but are not limited to the
following:

A. Body Harness – Only full-body harnesses shall be used. The use of a


body belt is prohibited.
B. Connecting Device – Shock-absorbing lanyards and lifelines
C. Anchorage – Anchorage point and anchorage connector

ii. Positioning
A positioning device is not a substitute for a personal arrest system and is limited
to use as system rigged to allow an employee to be supported on an elevated
vertical surface, such as a wall, and work with both hands free while leaning. It
must be noted that only a full-body harness shall be worn as part of a positioning
device system. Bodybelts are not acceptable.

iii. Suspension
Personal Suspension systems are used for window washing and painting and are
designed to lower and support a worker to perform tasks. The components of a
suspension system are:

A. Full-body Harness;
B. Workline;
C. Anchorage; and
D. Positioning Device such as a boatswains chair.
A boatwain's chair system is considered a single-point adjustable suspended
scaffold. Since the suspension system components are not designed to arrest a
free fall, a back-up fall arrest system should be used in conjunction with the
personal suspension system that would activate only if the worker were to
experience a free fall.

iv. Retrieval
Personal retrieval systems are used for confined space entry and on-entry
rescue. Personal retrieval systems consist of the following:
A. Full-Body Harness
B. Retractable lifeline/rescue unit; and
C. Tripod

v. Restraint
A restraint line is a device which is attached between the employee and an
anchorage point to prevent the employee from walking or falling off an elevated
surface. It does not support an employee at an elevated surface, but rather,
prevents the employee from leaving the elevated surface or work position.

1.08. Inspection & Maintenance

Equipment used as part of a fall protection system is

a) Inspected by the worker as required by the manufacturer before each use,

b) Kept free from substances and conditions that could contribute to deterioration of
the equipment, and
c) Re-certified as specified by the manufacturer. Inspections shall be carried out
prior to use and by a competent person, trained in the inspection of that
equipment and knowledgeable of the manufacturer’s requirements for inspection.

Equipment is removed from service and either returned to manufacturer or destroyed if:

a) it is defective; or
b) it has come into contact with excessive heat, a chemical, or any substance that
may corrode or otherwise damage the fall protection system, or
c) it has stopped a fall, and
d) equipment removed from service is not returned to service unless a professional
engineer or the manufacturer certifies it is safe to use.

The following criteria will be utilized when relevant to maintain all equipment in good
working condition.

Full Body Harnesses

1) Inspect before each use.


 Closely examine all the webbing to ensure that there are no burn marks,
which could weaken the material.
 Verify that there are no torn, frayed, broken fibres, pulled stitches, or frayed
edges anywhere on the harness.
 Examine the D-ring for excessive wear, pits, deterioration, or cracks.
 Verify that the buckles are not deformed, cracked, and will operate correctly.
 Harness should never have additional punched holes
 Check tongue/straps for excessive wear from repeated buckling.
2) Annual inspection of all harness will be completed by a competent person,
documentation will be maintained on file.
3) Storage will consist of hanging in an enclosed cabinet, to protect from damage.
4) All harnesses that are involved in a fall will be destroyed.

Lanyards / Shock Absorbing Lanyards

1) Inspect before each use:


 Check lanyard material for cuts, burns, abrasions, kinks, knots, broken
stitches and excessive wear.
 Inspect the snaphooks for hook, locks and eye distortion.
 Check carabiner for excessive wear, distortion and lock operation.
 Ensure that all locking mechanisms seat and lock properly.
 Once locked, locking mechanism should prevent hook from opening.
 Visually inspect shock absorber for any signs of damage, paying close
attention to where the shock absorber attaches to the lanyard.
 Verify that points where the lanyard attaches to the snaphooks are free of
defects.

2) Annual inspection of all lanyards will be completed by a competent person,


documentation will be maintained.
3) Storage will consist of hanging in an enclosed cabinet, to protect from damage.
4) All lanyards that are involved in a fall will be destroyed.

Snaphooks

1) Inspect before each use:


 Inspect snaphook for any hook and eye distortions.
 Verify there are no cracks, pitted surfaces and eye distortions.
 The keeper latch should not be bent, distorted, or obstructed.
 Verify that the keeper latch seats into the nose without binding.
 Verify that the keeper spring securely closes the keeper latch.
 Test the locking mechanism to verify that the keeper latch locks properly.
2) Annual inspection of all snaphooks will be completed by competent person,
documentation will be maintained.
3) All snaphooks involved in a fall will be destroyed.

Self-Retracting Lanyards

1) Inspect before each use:


 Visually inspect the body to ensure there is no physical damage to the body.
 Make sure all back nuts or rivets are tight.
 Make sure the entire length of the nylon strap is free of any cuts, burns,
abrasions, kinks, knots, broken stitches and excessive wear and retracts
freely.
 Test the unit by pulling sharply on the lanyard to verify that the locking
mechanism is operating correctly.
 If manufacturer requires, make certain the retractable lanyard is returned to
the manufacturer for scheduled annual inspections.
2) Monthly inspection will be conducted by a competent person with documentation
maintained.
3) Service per manufacturer specifications.
4) Inspect for proper function after every fall.

Tie-off Adaptors / Anchorages

1) Inspect for integrity and attachment to solid surface.


2) Annual inspection of all tie-offs and anchorages by a competent person with
documentation.
3) All tie offs and anchorages will be destroyed and replaced after a fall.

Mobile Elevated Work Platforms (i.e. Forklift, Scissors Lift etc)

1) Inspect before each use.

2) Inspect and service per manufacturer guidelines. Forklift, scissors lifts, and safety
nets will be inspected at the beginning of each shift in use. Structural integrity or
forklift basket will be checked per same schedule.
3) Annual inspection of forklift basket will be completed by competent person with
documentation maintained.

Horizontal Lifeline

1) Inspect before each use for structural integrity of line and anchors.
2) Annual inspection by competent person.

Guardrails

1) Temporary systems – Daily visual inspection will be completed by a competent


person.
2) Temporary systems – Weekly, a complete structural inspection will be completed by
a competent person.
3) Permanent systems – Annual structural inspection will be completed by a competent
person with future frequency of inspection defined based on conditions / control
present.

Storage and Maintenance of Fall Protection System

1) Never store the personal fall arrest equipment in the bottom of a tool box, on the
ground or outside exposed to the elements (i.e. sun, rain, snow, etc.).
2) Hang equipment in a cool dry location in a manner that retains its shape.
3) Always follow manufacturer recommendations for inspection.
4) Clean with a mild, nonabrasive soap and hang to dry.
5) Never force dry or use strong detergents in cleaning.
6) Never store equipment near excessive heat, chemicals, moisture or sunlight.
7) Never store in an area with exposures to fumes or corrosives elements.
8) Avoid dirt and build-up on equipment.
9) Never use this equipment for any purpose other than personal fall arrest.
10) Once exposed to a fall, remove equipment from service immediately.

9. Training

All employees who may be exposed to fall hazards are required to receive training on
how to identify such hazards, and how to minimise their exposure to them. Employees
shall receive training as soon after employment as possible, and before they are
required to work in areas where fall hazards exist.

A record of employees who received training and training dates shall be maintained by
Competent Person. Training shall include:

a) A review of current Singapore legislation pertaining to fall protection;


b) Information about the effect of a fall on the human body, including

i. maximum arresting force,


ii. the purpose of shock and energy absorbers,
iii. swing fall,
iv. free fall;
c) An understanding of what a fall protection plan is;
d) Fall protection methods an employee is required to use at the work site;
e) Identification of fall hazards;
f) Steps being taken to eliminate or control those hazards;
g) Correct procedures for erecting, maintaining, disassembling and inspecting fall
protection systems;
h) Use, operation and maintenance of personal fall arrest systems;
i) Correct procedures for equipment and materials handling, and storage, and
erection of overhead protection;
j) REI requirements for reporting incidents;
k) Emergency response procedures of the work site

Additional training shall be provided on an annual basis, or as needed when changes are
made to this Fall Protection Plan, or the legal legislation.

Personnel expected to conduct a site rescue will be trained in the procedures and
techniques to conduct such a rescue.

10. Incident Investigations

All incidents that resulted in injury to the workers and near misses, regardless of their
nature, shall be reported and investigated. All incidents shall be investigated as soon as
possible in accordance to REL-HSE-06 (Incident Reporting & Investigation) to identify
the cause and means of prevention to prevent future occurrences.

In the event of such an incident, this Fall Protection Plan shall be reviewed to determine
if additional practices, procedures or training should be implemented to prevent similar
incidents in the future.

11. Emergency Preparedness

A written emergency response plan with regards to falls shall be established in

In addition, some means of basic rescue capability that is capable of reaching a


suspended worker, will be made available at the work site and they may include:

a) Mobile Elevated Work Platforms (i.e. Forklift, Scissors Lift etc) with a competent
operator.
b) Ladders
c) Full body harnesses with leg loop extensions (i.e. suspension relief straps). These
attach to the body harness, providing foot loops into which a suspended worker
can place his or her feet and then raise the legs. Doing so allows blood pooling in
the legs to circulate. Using foot loops may help the worker to remain comfortable
until he or she returns to safe ground.

d) Rope rescue kit that will allow the suspended worker to be raised upwards or
lowered to safe ground. Use of this device does not require the suspended
worker to be conscious.
e) Equipping workers in certain situations with self rescue devices such as
specialized descanters that allow the suspended worker to remove themselves
from their lanyard and descend to safe ground using one of these devices.

12. Periodic Review

12.1 Evaluation

This Fall Protection Plan will be evaluated periodically to determine effectiveness.


The following criteria will be used to evaluate its performance:

a) Accident reports, number of accidents.


b) Management / staff compliance with Fall Protection Plan components.
c) Periodic on-site audits.
d) Staff feedback, interviews.

12.2 Changes to the Plan

Any changes this Plan will be made by Competent Person(s). This plan shall be
reviewed by Competent Person(s) as the job progresses to determine if additional
practices, procedures, or training are needed to improve or provide additional fall
protection. Affected employees shall be notified of changes to this Plan, and
retrained, if necessary. A copy of this Plan and all approved changes shall be
maintained at the project site by Competent Person(s).
