Relationship Disclosure Form

All SCI employees and potential candidates applying to SCI or currently undergoing a recruitment
process, are expected to disclose relationships with family members working for SCI at the current time.

All employees and/or candidates who are currently in the recruitment process are required to sign a
Relationship Disclosure Form. This is to enable the organization to protect its programs and individual
employees against conflicts of interest. This also helps in preventing the interference in staff
management and supervision, and helps in avoiding bias in decision-making.

Please complete the below form accurately and to the best of your knowledge and belief. You are
required to identify and declare any of the following types of relationships employed within SCI at any
Field location and programs:

 Spouse , Partner
 Mother, Father
 Sister, Brother
 Son, Daughter
 Cousins (First and second)
 In-laws (all types)
 Close Friends
 Previous working relationship; please
specify where did you work together
and for how long, along with your job
titles.

Once you complete the form with 100% accuracy, please sign it to confirm the details mentioned. Any
misleading information provided or undisclosed information on purpose may lead to a disciplinary
action in the future.

This form will be saved in SCI staff member personnel file, and for current candidates undergoing a
recruitment process; it will be part of the recruitment process documentation/file.

on SCI policies and procedures.

On exceptional basis, SCI will make impartial and sensible efforts to accommodate the hiring of relatives or people
with close personal relationships to SCI employees in a non-discriminatory way. This will be handled carefully and
closely on case-by-case basis with a specific approval to be sought from the Country Office Human Resources
Director (HRD) if it is a national post, and for international posts from the Regional HR Director (RHRD).
