Guide for Value Factor Rating


This form is divided into two parts: Key Result Area and Work Ethics/Attitude. KRA refers to quantifiable output of the employee and WEA
refers to the behavior and characteristics the employee has to exhibit in order to perform efficiently in his job. Please provide objective rating
using the 1-10 scale below:
1 - FAILED Employee not fit for the job
2 - LACK OF ABILITY Performance was consistently below expectations.
3 - POOR Employee is willing but unable even with supervision; Willing but unable
4 - UNACCEPTABLE Performance failed to meet expectations, and/or one or more of the most critical goals were not met.
5 - UNSATISFACTORY Failure to meet expectations but employee may be trained and needs constant supervision; Employee is willing and trainable
6 - AVERAGE Lower than Satisfactory; Non-Productive but trainable
7 - GOOD Ordinary kind of performance quality; With room for improvements given the proper training and supervision
