The failures on the pit wall have potency of the next movement. Geotechnical
Engineering should investigate it and make a report.
This report is to record and define failures to the pit slopes, interpretation of the hazard
potency to protect people working in the open pit.
Scope of this procedure applies to all MSM/TTN staff and contractors completing pit wall
failure report.

2 SAFETY

2.1 PPE Requirements

Minimum Required: Hard Hat

Steel capped safety boots
Safety glasses with side shields
Collared shirt

Optional equipment: Hand held radio communication

Gloves
Disposable ear plugs
Goggles
P2 disposable dust mask
Sunscreen and insect repellent

2.2 Hazard and Risk

The failure on the pit slope or wall has potentially dangerous mass movement. Always
be aware of overhanging rocks when working near any face. If a section of a wall
appears too dangerous, do not to work beneath it. An apparent dip and dip direction and
an approximate position will suffice in such situations. In some circumstances, it may be
possible to call the Production Supervisor and organise for the removal of the hazard.

3 PROCEDURE

 Only personnel inducted for the Mine area may undertake these surveys.
 Inform to Pit Supervisor before entering the failure zone.
 The Geotechnical Engineer, Geologist or Geotechnical Technician should undertake
the description.
 Complete the Pro-forma attached. CORP-GEO-FM-0001 Pit Slope Failure Report.
 Photograph the failure with a digital camera and attach in jpg format to Pro-forma
 Attach copy of survey plan prior to failure.
 Enter copy into Geotechnical Management System.
 Map all cracks and record sense of movement, survey the crest of the scarp and
undertake broad scale geological mapping.
