CDA Renewal of Accreditation

For Individual CPA

1. Latest Professional Regulation Commission (PRC) ID

2. Valid Individual Certificate of Registration with the PRC-BOA

3. Professional Tax Receipt issued by the local government

4. Valid Certificate of Membership in Good Standing with his/her PICPA Chapter

5. Certificate of Training equivalent to 24 hours of training attended on cooperative

updates and issuances and other related courses provided by CDA-accredited
institutions

6. Expired Certificate of Accreditation issued by the CDA

7. List of cooperatives audited

8. Sworn statement that the cooperative external auditor has engaged in at least
one activity for cooperatives that is free of charge (e.g. installation of book of
accounts, consultancy, etc.) as part of his/her social responsibility, and

9. Sworn statement by the Cooperative External Auditor that he/she has observed
the conditions prescribed under Sec. 5 of Memorandum Circular 2009-03
