Court of the nature of the charge against him and his rights under the Constitution, and he stated that he
understood the nature of said charge against him and his constitutional, statutory, and procedural rights,
and that he had fully discussed all aspects of his case with his attorney, , and he was completely
satisfied with the legal representation and advice and waived in writing and in open Court prosecution
of the offense by indictment,

By way of information, Trevor Elkins late of said County, did, while being a public or party official,
attempt to commit any theft offense, as defined in division (K) of section 2913.01 of the Revised
Code, when the offender used the offender's office in aid of committing the offense or permitted or
assented to its use in aid of committing the offense.

All of the above conduct being contrary to the form of the statute in such cases made and provided,
and against the peace and dignity of the State of Ohio.
