SUB: 2 nd WARNING LETTER ABOUT ABSENTEEISM POLICY

Dear Jatin Zalavadiya ,

You are an important member of our PBN chapter so it is my duty to inform you of absenteeism
Policies at PBN.

As per absenteeism policy of PBN, it is observed that you were not present in last meeting. As you are
aware about 4 absenteeism policies of PBN meeting, this was your 3rd absent in 6 month rolling
period, so kindly note that absenteeism will affect your monthly MPR-Member Performance Report
and also your membership will be discontinued after 4th absent in your 6 month rolling period.

Your absenteeism is loss to members of pbn chapter and to you as well.

We value your contribution towards chapter so kindly note it and remain present in next meeting.
