Summary of Conversations with Candace About Independence and Autonomy

Candace and I discussed what her goals would be and she was able to come up with a few that she felt
would help her in her day-to-day life. It was important to her that she learned how to save money and
understand how to create a budget for her spending. She thinks she would like to get her license one
day in the future but she is too nervous at this time and doesn’t feel that she is ready. She would like to
learn a little more about riding the bus so she feels comfortable enough if she needed to go somewhere
on her own. She tries to eat healthy and excises with her mom but she would like to learn a little more
about healthy eating habits.

Summary of Conversations with Candace About Educational Pathways

Candace and I had serval conversations about whether she wanted to continue her education,
and although she really enjoyed her time at Mohawk, she is ready to graduate and take a break
from school. Updating her resume is something she will need help with for when she is ready to
apply for a job. Since she loves being around children, she thought it would be a great idea to
have her first aid and CPR in case there was ever an emergency. She would also love to attend
some arts and crafts workshops to help get some ideas of activities she could do with other kids.
We discussed exploring the Early Childhood Education Program that she may be interested in
down the road, so I researched a few options for that as well.

biggest things is, being around children. Candace would love to work at a daycare one day so
we researched job opportunities that would allow her to be around children and gain experience
working with them. She loved doing her co-op at Tiny Hoppers and would love to get hired there

Summary of Conversations with Candace About Recreation and Leisure
Candace is very active and loves the outdoors so we had fun coming up with ideas for recreation
and leisure. Some of them she has done before and was excited to try them again with family
and friends. Candace is very social and loves hanging out with her friends. She always has a
smile on her face when she talks about her friends and family. We have talked about many
adventures that she has been on so when it came to choosing fun things for peer connections,
she had many great ideas. The one she is really excited about and would love to do is travel to
Mexico! I hope Candace is able to go there one day because I know she will have a great time!

20
OUR JOURNEY
It has been an absolute pleasure supporting Candace. She certainly made this experience one
that I will never forget. I considered myself lucky when I got to work with her throughout first semester
so when I found out that she was going to be my partner again for fourth semester, it was like winning
the lottery. I had the opportunity of getting to know Candace very well and learning so much about her.
Although we never had the chance to meet in person it was as though I have known her for years. We
were so comfortable with each other from day one. I truly hope Candace and I have the chance to meet
in person one day!
