Comment: THIS IS RE-INSPECTION 1. THE FIRE ALARM PANEL IS IN SUPERVISORY MODE. AT THE TIME OF THIS
INSPECTION THE FIRE PUMP IS BEING WORKED ON AND IT IS CAUSING THIS FAIL TO SHOW UP ON THE
MONITOR SCREEN. AT THIS TIME ALL LIFE SAFETY COMPONENTS ARE OPERABLE. THE ALARM PANEL
WILL RING OUT AND ALERT WHEN ACTIVATED. THE FIRE EXTINGUISHERS NEED TO BE SERVICED FOR
MAINTENANCE .
RE-INSPECTION TWO. AT THIS TIME THE FIRE ALARM PANEL IS SHOWING TROUBLE AND SUPERVISORY
MODE. THE PANEL READS THAT IT IS A BEAM DETECTOR DEFAULT. I DID A FIRE ALARM TEST BY
ACTIVATING A PULL STATION TEST. THE FIRS FLOOR THE BELLS DID NOT SOUND AS LOUD AS THE
UPPER FLOORS. ALSO, THE STROBES DID NOT ACTIVATE. YOU CAN STILL HEAR THE ALARMS
THROUGHOUT THE BUILDING HOWEVER THIS NEEDS TO BE ADDRESSED.

DCMR 12H F-113.1 Right of Appeal. Any person directly affected by a notice or order issued under this Fire Prevention Code
shall have the right to appeal to the Office of Administrative Hearings, pursuant to the Office of Administrative Hearings Act,
effective March 6, 2002 (D.C. Law 14-76; D.C. Official Code §2-1831.01 et seq. and regulations promulgated thereunder. The
appeal shall be filed within ten (10) days of the date of service of the notice or order. An appeal shall be based on a claim that the
true intent of this code has been incorrectly interpreted, the provisions of the code do not fully apply, or the requirements of this
code are adequately satisfied by other means. Appeals of notices (other than notices pursuant to Section F-110H (Unsafe
Conditions) or section F-111H (Emergency Measures) shall stay the enforcement of the notice until the appeal is heard by the
Office of Administrative Hearings.
Failure to remedy said violations will subject you to the penalties as prescribed by Section 112.2 and
F-112.3 of the International Fire Code as amended by the D.C. Fire Prevention code Supplement (DCMR

907.8.5 - Inspection, testing and maintenance: The building owner shall be responsible to maintain the fire and life
safety systems in an operable condition at all times. Service personnel shall meet the qualification requirements of
NFPA 72 for inspection, testing and maintenance of such systems. Records of inspection, testing and maintenance
shall be maintained.

exhaust systems, and smoke and heat vents shall be maintained in an operative condition at all times, and shall be
replaced or repaired where defective. Nonrequired fire protection systems and equipment shall be inspected,
tested and maintained or removed.

or monetary fines will follow.

Comment: Both basement storage rooms has openings in the wall near the ceiling.

703.1 - Maintenance: The required fire-resistance rating of fire-resistance-rated construction, including, but not
limited to, walls, firestops, shaft enclosures, partitions, smoke barriers, floors, fire-resistive coatings and sprayed
fire-resistant materials applied to structural members and fire-resistant joint systems, shall be maintained. Such
elements shall be visually inspected by the owner annually and properly repaired, restored or replaced where
damaged, altered, breached or penetrated. Records of inspections and repairs shall be maintained. Where
concealed, such elements shall not be required to be visually inspected by the owner unless the concealed space is
accessible by the removal or movement of a panel, access door, ceiling tile or similar movable entry to the space.
Openings made therein for the passage of pipes, electrical conduit, wires, ducts, air transfer openings and holes
made for any reason shall be protected with approved methods capable of resisting the passage of smoke and fire.
Openings through fire-resistance-rated assemblies shall be protected by self- or automatic-closing doors of
approved construction meeting the fire protection requirements for the assembly.
