Category of Incident – NM (repeated incident)


❖ Description of Incident
A Pick and Carry crane was engaged in shifting of concrete blocks of weight 2.5 T. While lifting the concrete block,
the wire rope of the crane hook got slipped / detached from the U clamp and the concrete block fell on the ground
from the height of 0.5 Metres. Fortunately no one got injured in this incident.

❖ Causes: Crane wire rope slipped from U- Clamp


Direct Cause(s):
• Concrete block fall on ground due to wire rope slipped from the U clamp.

Underlying Cause(s):
• Wrong orientation of the U-Clamp - The saddle of top and bottom U-clamp was fixed at the dead end of
wire rope. The centre U-clamp saddle was facing live end of wire rope.
• Lift permit was not obtained by site engineer
Root Cause(s):
• U-Clamp verification for its correct orientation was not done during the periodical Inspection.

❖ Preventive measures to be taken to STOP recurrence of such incident:

• Regular inspection and maintenance of the crane.


• Lift permit to be obtained before lifting activity.
• Awareness Training to be conducted.
• Ensure the availability of over hoist limit switch.
• Number of U-clips and orientation of the U-Clip shall be
provided as per the standard (IS 2361 – 2002) – Refer
Fig 2 (Fig -1)

• Nyloc lock nut shall be used in U-Clamping.


• Arrest the movement of hook while travelling without
load (Fig -3)
• Wedge lock arrangement can be used to replace U-
clamping.

• To avoid frayed wire rope, wire rope end treatment shall be done (wrap the end with thin soft wire/bending
wire) – Refer Fig 4
