Agreement number and date: 18/2018-19, dated 07.03.2019.

Status of Agreement: 07.03.2019 to 06.03.2019 (Two years) Agreement period is lapsed by

06.03.2021 and EOT was sanctioned on 28.05.2021 for a period of 2 years (i.e., 31.03.2023)

Physical Progress: As on date XX physical progress is observed at Yemmiganur ULB. Progress of

works are very slow. The contracting agency may please be notified to speed up the work by
deploying sufficient work force and procuring DI Pipes Immediately to expedite the work
progress.

Financial Progress: (0 %)

RA Bill 1 to RA Bill 7, Gross amount of Rs.18,73,56,815.00 and Net amount of

Rs.15,02,86,263.00. This bill is uploaded in CFMS and same been paid on dated 04.04.2022.

Details & Observed remarks by structures wise:

1. 10m dia. Intake well cum pump house in Gajuladinne project – Structural designs are
approved. NOC for land to be received from water Resources Department vide lr.No.
DB/D2/282/Amrut 2016-20/TDP-Mty, dated: 14.03.2021.
Status of work: No design and drawings been submitted from contractor end for
approval
2. 18 MLD WTP in Guddikal road : Land been handed over to contractor from EE(Public
health)-Kurnool on dated
Status of work: Work is yet to be grounded by the agency.

3. Raw Water and Clear Water DI Pumping Mains (DN 200 to DN 500 - Length - 31.40 km,
as per BOQ): Hydraulic designs got approved and Procurement of pipes not yet started
by agency.

4. 1000 KL ELSR In Zone-4 at Pagidala Main Road: (Value of work done: 67,20,000/-)

Concreting for columns completed up to 3rd brace beam. It is observed that 40KL ELSR
Existing near the construction area is looking in dilapidated condition. This ELSR may be
dismantled or if it not possible to dismantle it, then proper safety precautions may be
taken to avoid workforce working near this existing ELSR.
Reason for delay in work progress: Less work force is deployed at site, which in turn
will cause delay in work progress & In connection to that work force are regularly
leaving sites as payment is being delayed.

5. 1300 KL ELSR In Zone-1 at BSNL Office: (Value of work done: 79,56,000/-)

Concreting for 1st lift columns completed above the 3rd brace beam. Due to non-
availability of work force at site, progress of work is slow.
Tree cutting is observed at the time of site clearance (3 No’s), in this regard, evaluation
report received from DFO on 16.12.2020. Auction is yet to be conducted by
Nandikotkur Municipality.

6. 900 KL ELSR In Zone-2 at BRR Nagar: (Value of work done: 59,40,000/-)

Concreting completed for the 2nd brace beam.
90 KL ELSR is existing near the construction area which is in dilapidated stage. On this
issue PMC has given mails to PHMED and Agency-loCT to take proper safety protection
works to avoid any mishap/accident at site. As per the SE-PH instructions, Municipality
of Nandikotkur has taken action by stopping the water supply from the Existing ELSR
and given the direct pumping system to the consumers under Existing ELSR.

Severe Honeycombs were observed at Bottom portion of 1st brace beam. Rectification to
be done as per clause 15.7 of SP:24-1983, filling the voids with epoxy grouting. This
 issue raised by PMC 18.11.2021, 01.12.2021, 12.12.2021 & 20.12. 2021.Rectification is
to be pending.

Tree cutting is observed that in the time of clearance of site (2 No’s), in this regard,
evaluation report received by the DFO on 16.12.2020. Auction yet to be conducted by
the Nandikotkur Municipality.

7. 1100 KL ELSR In Zone-3 at Government School Shikaripet: (Value of work done:

50,82,000/-)
Concreting completed up to GL Brace beam.
Reason for delay in work progress: Work is on hold for 8 months as per instructions
received from the Honorable high court orders on 16.08.2021 (i.e., any construction
activity in the school’s premises has to be stopped which were unrelated to schools)

 Land Issues and Present Status: The owner of proposed WTP site under APUWS&SMIP
Nandikotkur scheme is Ms. Palla Narasamma, W/o Chowdaiah agreed to sell 1.83 acres
of land near midtoor road in the year of 2019. Proposed 11 MLD WTP private land
acquisition status as on 23.08.2021 is under scrutiny at O/o District Revenue Officer,
Kurnool. General award is approved & received proceedings from the district collector
Kurnool, Acquisition is in progress for the land.
 Status of structural designs to be furnished by the contracting agencies and Reasons
for delay: It is to submit that the Zone -I, II, III, IV ELSRs and Intake well structural
designs are approved as on 23.08.2021. In case of 11MLD WTP, Except Clear water
reservoir remaining components of WTP structural designs are under scrutiny at O/o SE-
PH Office at Ananthapur Circle.

 Construction difficulties, if any, in case of Intake wells: Proposed intake well capacity of
10 m dia with 50m approach bridge at Alaganuru balancing reservoir which has
minimum draw level – 248.50 ft, live storage – 2.965 TMC, FRL – 869.42 ft with 7.50 Kms
of earthen bund. There will be no difficulties estimated in the construction of proposed
intake well when the Form work, Dewatering, excavation, construction materials
approach methodologies to be upgrade w.r.t. safety & Covid – 19 hygiene aspects. NOC
for land to be received (Letter number-DB/D2/282/Amrut 2016-20/TDP-Mty Dated
14.03.2021).

 Establishment of Full-Fledged laboratory: IOCT agency has been established the full-
fledged laboratory at Nandikotkur RMC batching plant at aalur road. It is requested to
instruct the agency to establish permanent set up for the laboratory and deploy all the
equipment as per loCT-Agreement condition (Clause- 11.1.3).

 Deployment of adequate Workforce: Contracting Agency deployed staff at grounded

works, but these deployed staff are not sufficient as per agreement (including Technical,
Safety and Environmental).

 Submission of Construction Schedule: IOCT agency has been submitted their

construction schedule through EE PHED Kurnool to SE PHED Anantapur circle. It is
pending for approval at O/o the SE PHED Anantapur circle.

 Procurement of Materials from the Factories: Total quantity of pipes to be procured, as

per BOQ, at Nandikotkur is 31.40Km. Agency may please be instructed to expedite the
Procurement & Laying of pipeline for physical and financial progress.

 Design Mix and its approval: The Mix Design from Bureau veritas as (India) private ltd.
laboratory for M30 Grade pumpable & non-pumpable concrete got approved.
 Batching Plant and its approval: The establishment of RMC Batching Plant for
Nandikotkur & Atmakur ULBs at allur road of capacity 30 Cubic meter per hour has been
established on 21.12.2020 & Demobilization of RMC batching plant on 16.03.2022, from
Nandikotkur ULB in the middle of the project execution.

 Stacking of Reinforcement: It is observed that, certain sites at ULB, Reinforcement is

placed directly on soil and in some sites Placed over wooden bullies, which will not bear
the weight of reinforcement and made contact with earth. Due to this improper
stacking, every chance of reinforcement got rusted. Stacking of reinforcement should be
done as per clause-4.13 of IS: 4082-1996.

 Maintenance of Registers at site: Pouring register, DPR, Site Order book for each site
and Quality reports of construction materials are maintaining at Nandikotkur ULB by
PMC ground staff as well as IOCT staff.

 Maintenance of all safety & Environmental measures: - It Is observed that safety

measures such as Barricading, Sign Boards, Caution Boards, PPE to all workforce, First
Aid box and issue of Public Notice are not being maintained at many sites. Barricades
are not placed at sensitive areas (Traffic area, Residential Area, School). Labour working
at heights are not being used safety belts and safety nets are also not provided at
ELSR's.

For tree cutting auction is pending at Municipality of Nandikotkur, for fallen trees
(5no’s) at 1300KL ELSR BSNL Office (2no’s) & 900KL ELSR BRR Nagar (3 no’s). In this
regard evaluation report received by the district forest officer on 16.12.2020.

 Labor management procedures and provisions: As per C-ESMP report includes Labor
license, Labor screening and labor Insurance (ECA 1923 and other insurances) IOCT
agency is not maintained relevant data as per agreement condition. In case of labor
camps IOCT agency are provide temporary residential accommodation for labors with
arrangements of potable water and sanitation facility.

 Status of Council Resolution: The Council Resolution has been approved by Municipal
Commissioner, Nandikotkur Municipality on 28-09-2020 with reference of CR.no
380/2020 Dated: 28-09-2020. For the proposed 4 No.’s ELSRs sites, Owner of the Lands
i.e., Nandikotkur Municipality sanctioned the consent for Zone I, Zone II, Zone III & Zone
 IV ELSR sites and the same has been handed over to the Executive Engineer, Public
Health Division, Kurnool on 14.10.2020.

 Status of GRC Formation: The Grievance Redress Committee has been formed on
17.07.2020 at Nandikotkur ULB as per instructions given letter no.
2233/APUFIDC/EAP/AHP/2015 dated 12-11-2018 of the managing director, APUFIDC,
Guntur. Yet to conduct the GRC meeting till date, due to smooth execution of IOCT
works at ULB Level.

 Status of CMU Formation: The City Management Unit has been formed on 18.03.2020
at Nandikotkur ULB with Municipal Commissioner, Nandikotkur Municipality as
chairman w.r.t. proceedings ROC. No. 605/E1/2020 dated 18.03.2020.
Minutes of meeting held on 01.02.2021 in the office of the Municipal commissioner,
Nandikotkur municipality on the progress of the AIIB scheme by the City management
unit, along with MC,Nandikotkur and chairman & all members including PHMED
Engineer.

Package – 2 (Distribution Package – NCT)

Name of the Contracting Agency: M/S Indian Hume Pipe Limited.

Value of Contract: 41.06 Cr

Agreement number and date: 17/2018-19, dated 26.02.2019.

Status of Agreement: 26.02.2019 to 25.02.2021 (Two years). Agreement period is lapsed by

25.02.2021 and EOT was sanctioned up to 25.08.2021 is also lapsed and again EOT was
sanctioned up to 24.02.2023.

Physical Progress: Work is yet to be grounded.

Financial Progress:

RA Bill 1: Gross amount of Rs.1,85,13,708.00 Net amount of Rs.1,76,02,336.00. This bill ls

uploaded in CFMS on 09.12.2021, vide Bill No. 2021-227026. Amount yet to be paid.
Status of Hydraulic designs: Hydraulic Designs of distribution networks are approved .
(Zone-1, Zone-2, Zone-3 & Zone-4)

Procurement of Materials: Quality Assurance Plans of DI Pipes, HDPE Pipes, HDPE Specials and
House Service Connections are approved. Agency is procured of HDPE Pipes for Nandikotkur
ULBs for a length of 35KM & Factory inspection completed for the HDPE Specials on Dated:
13.02.2022. yet to be received the inspection material from Manufacturer.

Deployment of Technical staff: Contracting Agency deployed staff at ULB, but these deployed
staff are not sufficient as per agreement. This has to be expedited by the contracting agency.
The contracting agency may please be notified once again, in this regard.

Narrow Gullies and Congested Areas: The existing HSCs of No.’s 4697 at Nandikotkur ULB with
25.20 km as distribution network. The estimated HSCs of 11645 under APUWS&SMIP at
Nandikotkur scheme 108 km. narrow gullies and congested areas are observed at kota veedi,
byreddy nagar and Sai Baba peta. The laying of Clear water pumping main at Saibaba peta &
kota veedi and the excavation work, construction material approach to the byreddy nagar ELSR
site (pagidyala main road) may be difficult.

Areas where possibility of encountering rock during excavation: The areas where possibility of
encountering rock during excavation at Nandikotur ULB is at kota veedi, byreddy nagar and Sai
Baba peta. As per Letter No.DB/AEE/AIIB/WSIS/2018 Dated 31.07.2021 of SE PHED Anantapur
Circle addressed to NCT agency, the NCT agency was requested to start the work immediately
as per deliverable and agreement conditions. Hence once the NCT agency may start the work
the possibilities of encountering the rock may be evaluated.

Water logging Areas – The water logging areas at Nandikotur ULB is null.

Status of hydraulic designs to be furnished by the contracting agencies and Reasons for delay:
The NCT hydraulic designs are approved as on date 23.08.2021.

Submission of Construction Schedule: NCT agency has been submitted their construction
schedule through EE PHED Kurnool to SE PHED Anantapur circle. It is pending for approval at
O/o the SE PHED Anantapur circle.
