STATUTORY DECLARATION

I, ANYWAR DENIS PETER of C/O P.O BOX 11 Refugee Law Project-

Bweyale, Kiryandongo, Uganda do hereby solemnly take oath and declare as
follows;

1. THAT I am a male adult South Sudanese of sound mind, registered as a

refugee in Uganda by the Office of the Prime Minister, and I make this
declaration in that capacity.
2. That I make swear this Statutory Declaration as the next friend of
OCHANA DALTON RHEMUS (a minor).
3. THAT upon verification as a refugee in 2018, JOHN DOLO OBONG was
issued with a Proof of Verification wherein the names of the Minor was
incorrectly written. (A copy of the Proof of Verification is hereto
attached and marked Annexture “A”).
4. THAT the name DALTON DOLO OKENY appearing on the Proof of
Verification are not the minor’s true names.
5. THAT the true name of the minor is OCHANA DALTON RHEMUS and
his school report card reflect the same name. (A copy of the school
report card is hereto attached and marked Annexture “B”).
6. THAT I make this Statutory Declaration in rectification of the minor’s
name.
7. THAT I make this declaration verily knowing the contents to be true and
correct to the best of my knowledge.
