DECLARATION OF REVOCATION OF TRUST

KNOW ALL MEN BY THESE PRESENTS:

________________, do hereby cancel and revoke any trust over ______(_______) share
of stock (the “Share”) in _____________________ (the “Corporation”), a corporation
duly organized and existing under Philippine laws, executed in favor of __________
(“Trustor”). This revocation is immediately effective on _____________.

In revoking the Deed, I hereby absolutely and unconditionally return and transfer
the Share registered and issued under my name unto the Trustor being the beneficial
owner or to such person or entity as the Trustor shall name and all rights, title and

thereon, free of all liens and encumbrances.

Further, I hereby constitute the Corporate Secretary with full authority to register

name to that of the Trustor or his assignee.
