What Went Well?

1. The Delivery Team (Mrs. Ma. Joan H. Villanueva) arrived at the Barangay Carmen Covered Court at
1:10 PM with the vehicle provided by the barangay. The same vehicle delivered back the box with learners’
bags and other materials such as disinfecting paraphernalia.
2. IATF and Managerial Protocols: The entrance and the exit were posted properly to make sure that there
will be no plausible contact between parents and teachers. The team was divided into two: Ms. Mrs. Ma.
Joan H. Villanueva (disinfection, checking of forms and other paper works) and Ms. Ma. Judy Gracia G.
Tomnob (distribution). All parents/guardians have their own ballpen, followed the IATF protocols:
disinfection, wearing of face masks and face shields, maintained one to two meters social distancing.
3. The parents and the members of the barangay are very supportive during process of retrieval and
distribution.
4. There is a 100% success rate in the delivery of learning bags.
What Went Wrong?
1. There are students who are unreached due to lack of information both in the learner’s bags and student’s
directory.

How We Resolved the Issues?

1. The team called the attention of the adviser to properly address the issue. The adviser took the bags upon
adviser’s request.
