RELEASE AND QUITCLAIM

That I, _________________, of legal age, and a resident of _____________, having

resigned from my employment with ______________________. (“The Company”) effective
_____________________, hereby acknowledge receipt from the Company based on the
releases specified below:

due me, including but not limited to salaries, 13th month pay, leave benefits, retirement,
redundancy, separation benefits from the Company arising from my employment and the
cessation of such employment. I acknowledge receipt of all separation benefits due me from
the Company, its affiliates, officers, directors or assigns and hereby waive and relinquish all
rights that I may have to any further amounts that may be accorded as separation benefits.

In consideration of the above-mentioned payment, I discharge and, by these presents,

for myself and my heirs, successors and assigns, release and discharge the Company, its
officers, agents, successors, and assigns, from any and all cause or causes of actions, sum or
sums of money, accounts, damages, claims and demands, in law or in equity, which I had, now
have and which I, my heirs, successors and assigns shall or may have against the Company.
My intention herein is to completely and absolutely discharge and release the Company, its
officers, agents, successors and assignees from all liabilities arising, wholly or partially, directly
or indirectly, form my employment. I freely declare that during the entire period of my
employment with the Company, I was paid all compensations and received all benefits and
privileges to which I was entitled under Philippine laws and under the policies of the Company. I
also acknowledge that I have not been and am not suffering from any work-connected illness or
disability. Should I hereafter find that I have been entitled to further amounts, I receive the
above sum of money as full and final satisfaction of any and all such amounts. I further
manifest that the payment of the above sum of money shall not be taken by me, my heirs,
successors and assigns as a confession and/or admission of liability on the part of the Company
for any cause or demand that I may have against the Company.

I commit to keep secret all confidential information given to me or gained in confidence

during my employment with the Company. I shall not, in any fashion, form or manner, unless
specifically consented to in writing in advance by the Company, either directly or indirectly use,
whether for my own purpose or benefit or that of any third party, divulge, disclose or
communicate to any person or corporation, any information of any kind, nature or description
concerning any matter affecting or relation to the business or affairs of the Company, including
without limiting the generality of the foregoing, the names and the practices of its customers,
customers list, code designating customers or codes for other information, its marketing
methods and related data, marketing and technical tools, computer based and manual systems
specifically designed for the Company, credit information of the Company or its customers, the
names of its vendors or suppliers, costs of material, the prices it obtains or has obtained or at
which it sells or has sold its products or services or other written records used in the Company’s
business, compensation paid to employees and other terms of employment, or any other
confidential information, of about, or concerning, the business of the Company, its manner of
operation, or other confidential data of any kind, nature or description.

I hereby declare that I have read and understood this document before signing it and the
release and quitclaim hereby given is made willingly and voluntarily and with full knowledge of
my rights under the law.
