NURSING
CARE PLAN
GROUP 2
Introduction
This work shows a real-life scenario. Family nursing diagnosis is a clinical judgment regarding existing or
probable health problems and life processes that occur in the family. It allows us to reconcile theory and practice.
It is validated when the defining traits, relevant factors, and risk factors uncovered during the assessment are
clearly defined and linked to it. Logical reasoning, in which relationships are formed to arrive at reasoned
judgments, distinguishes the planning process. This presentation focuses on the family's health needs and
problems to be able to establish priorities, select appropriate and or implement the nursing actions and evaluate
each member of the family.

FAMILY NURSING CARE PLAN | GROUP 2

The family nursing procedure will result in a care plan that describes each client's needs and care. It is a
collection of actions that we select and implement from a variety of options after careful consideration and
evaluation of alternatives in order to address identified family health and nursing issues.

The particular reason for choosing this family is, we are able to identify their health problems and provide
solutions through health education and treatments. The family will be able to comprehend and learn about
All the data included in this case presentation was collected from a personal interview which was conducted by one
of our members inside of the client's home. Social distancing and safety protocols are implemented during the
interview.

DATA INITIAL BASED NARRATIVE
Following the approval of the data acquired, we have come up with the following problem. Poor Home/Environmental
Sanitation, Sedentary Lifestyle, Poor Ventilation, Improper Drainage Facility, Inadequate Living Space and Presence of
Vector .

Additionally, the abovementioned 6 discovered problems have been subjected to a rating procedure that tries to
determine the most important problem priority. The first of the priorities is the Improper Drainage Facility with the
total score of 4.67. It is a health threat because it can spread infections like dengue fever and malaria, putting the
family's health at risk. Second is the Poor Home/Environmental Sanitation with a total score of 3.84. It can be a health

threat because of poor disposal of trash, by getting a bacterial infection using water from an open drum of water, and
It can also be a breeding site for mosquitos. Thirdly, the Presence of Vectors with the total score of 3.67. It is a health
threat to the family because there is a possibility for the family to acquire vector borne diseases. Next is Sedentary
Lifestyle with a total score of 2.84. It is considered a health threat that may lead to cardiovascular diseases especially to
Mrs. Lourdes. Second to the last is Poor Ventilation with the total score of 1.97. It can be a health threat because
inadequacy of ventilation may increase hazard vulnerability of transmission of bacteria viruses. Lastly, the least and
