ESSENTIAL NEW BORN CARE

[ OFCOURSE BEFORE WE START EH BATIIN MUNA NATIN SI MA’AM AND INTRODUCE NATIN SARILI NATIN,
AFTER THAT IS PROCEED NA TAYO SA PROCEDURE]

1. So First thing first, we will do is to remove all the jewelries so that there will be no distraction during the
process
2. And assuming that we are already wearing scrub suit, mask and cap
3. Next is we will be preparing the materials that will be needed for the procedure
 Gloves, dry linen, bonnet, syringe, plastic clamp, instrument clamp, scissors, kidney basins
 And in separate sequence, we will be preparing the eye ointment, stethoscope, vit. K, Hepa B and
BCG vaccines, cotton balls, tape measure and thermometer.
4. Now that we’ve already prepare the materials we’ll be going also to prepare a clean newborn
resuscitation area and after that;
5. We will be checking if all the equipment that we will be using are clean, if it is functional and within easy
reach
6. So we will be doing hand hygiene to lessen the spread of microorganisms or any infectious diseases
7. We will be also donning 2 pairs of sterile gloves
8. And we are going to place dry linen on the mother’s abdomen in preparation for the drying of the baby
9. [ ditto is we’ll be performing ung gaagwin natin kay Ma’am Che so dalawa tayong mag aassit sa pagpapa
anak and we’ll follow the up and down pattern sap ag pull kay baby]
10. [ here ako magsasabi: time of delivery 8:00 am babae po si baby !!, and then irerecord ko

Immediate newborn Care [ time bounded: 30 minutes]

1. We’ll be putting the newborn baby on top of the draped abdomen of the mother [ilalagay nasi baby on
top of the abdomen ni mother tapos dry na si baby within 5 seconds]
2. So using dry and sterile gauze, I will be drying the baby starting from the ;
 Mouth [outer mouth lang ang punasan]
 Nose [ wipe lang]
 Eyes [ two hands gamitin mo, inner and outer tapos dapat gentle]
 Whole of the face but as always gentle lang
 Ears then pababa sa neck
 Trunk
 Back
 Arms
 Legs
3. We will be doing tactile stimulation in order for the baby to cry by just simply rubbing the baby’s back or
soles of the feet
4. [check the breathing of the baby while drying] so the breathing of the bay is just normal
5. [assess the newborn using APGAR scoring ]
 for the ACTIVITY – 2 POINTS
 PULSE – 2POINTS
 GRIMACE- 2 POINTS
 APPEARANCE- 1 POINT
 RESPIRATION – 2 POINTS, WITH THE TOTAL SCORE OF 9 POINTS MEANS YOUIR BABY IS
NORMAL
1-3 MINUTES

6. We will be promoting skin to skin contact between the mother and newborn
 So we will just remove the wet cloth to start skin to skin contact
 we will place the newborn into prone position on the mother’s abdomen or chest
 and we’ll just cover the baby with dry cloth and head bonnet
 so we’ll just reevaluate the newborn using Apgar scoring [ assess the baby in cephalon- caudal
manner]
 we’ll just continue the skin to skin contact of the mother and the baby
7. so we’ll just going to remove the soiled pair of our gloves before we handle the newborn’s cord.
8. [ properly- timed clamping and cutting]
 We’ll just palpate and wait for the pulsations to stop
 Next is we are now going to clamp the cord with the umbilical clamp 2cm above the base of the
cord. So we will not be going to milk the cord.
 [ apply forceps 5 cm away from the base of the cord]
 [then I cut na ung cord using sterile technique
 And of course we are going to check for the presence of 1 umbilical vein, 2 umbilical arteries and
if there are any bleeding near the base of the cord

NEXT IS FOR 15-90 minutes

1. We are going to promote early breastfeeding and we will be observing for feeding cues and we will
nudge the baby to initiate feeding
2. And of course we will support the mother for proper positioning and attachment

AND LASTLY FOR THE NON- TIME BOUNDED PROCEDURE

1. We will be going to administer Crede’s Prophylaxis [ on the bed na ata ni baby ito]
 First we open the newborn’s eye and gently hold the upper lid upward with the index finger and
lower lid downward with the thumb
 And then squeeze the eye ointment to deposit 1 cm of eye medication on the lower lid, so we are
going to apply the drug from the inner to outer canthus.
 And we will do the same thing on the other eye
2. Next is we will be placing an ID band barning the name of the baby and the mother on either side of the
wrist or ankle of the newborn, but we are going to put it on the ankle.
3. Now we are going to do anthropometric measurement for the baby
 First is we are going to get the height of the newborn [ ipaside muna si baby gently tapos ilagay
ung tape measure kay baby, head to toes just watch nalang ung na video mo bveh bka mas
malinaw] then sasabihin ko as circu nurse : female baby, 50 cm
 Head circumference [ occiput area, above eyebrows] sasabihin ko: head circumference, cm
 Chest circumference [ slide sa likod ni baby ung tape then itapat sa axilla and then measuer from
the nipple line] sasabihin ko: 30 cm
4. Next is we will be getting the baseline core temperature of the new born
5. We must first check for the anal patency of the newborn by inserting slowly the tip of the NGT tube into
the anus while positioning the newborn appropriately
[ circu Nurse: 37.8 degress celcius
6. Next we are going to check for the baby’s Weight [lapag mo carefully sa weighing scale with clean linen]

7. So next we do thorouigh physical exam [ I check lang natin ung buong physical body ni baby to observe
any abnormalities
8. And next we will be going to administer VITAMIN K to the newborn, intramascular into the lateralis
muscle, so the dose will depends on the weight of the baby. And then next is HEPA B, will be administer
on the right vastus lateralis muscle
9. And lastly we will be putting on the baby’s clothing and we will wrap the baby
[ and ayusing natin mga nagamit na mga materials and perform hand hygiene]
