CHATTEL MORTGAGE

KNOW ALL MEN BY THESE PRESENTS:

This CHATTEL MORTGAGE, made and entered by and between:

__________________ of legal age, Filipino, married to _______________ and a resident

of ______________, and hereinafter referred to as the MORTGAGOR,

- and -
______________________, a Cooperative duly registere in the ________________, doing
business at Ground Floor, ______, represented herein by ___________, as Chairman, of legal age,
Filipino, and a resident of No. 5, Zamables St., Rolling Hills Subd., Bacaca, Davao City, and hereinafter
referred to as the MORTGAGEE,

WITNESSETH:
          

THAT the MORTGAGOR does hereby convey, by way of Mortgage, unto the MORTGAGEE
certain personal property presently in the possession of said MORTGAGOR situated in Davao City and
more particularly identified and described as follows:

THAT the mortgage is given as security for the payment to the said MORTGAGEE of certain
promissory note for the principal sum of SIX HUNDRED THOUSAND PESOS ONLY (P600,000.00)
Philippine Currency, with interest of 22% per annum, according to

the terms thereof, copy of the promissory note hereto attached and forming part of this
document:

THAT the condition of this mortgage is such that if the MORTGAGOR, his/her heirs, executor or
administrator shall well and truly pay or perform the obligation above stated according to the precise
terms thereof, then this mortgage shall be null and void; otherwise, it shall remain in full force and
effect and be enforceable in the manner provided by law.

AFFIDAVIT OF GOOD FAITH

We severally swear that the foregoing CHATTAL MORTGAGE is made for the purpose of
securing the obligation therein specified and for no other purpose, and that the same is a just and
valid obligation and not one entered into for the purpose of fraud
