REPORT ON MOCK DRILL

 The mock drill is conducted on 22.03.07 at 13.45


 The location was ware house-I.
 A chemical drum which was stored inside caught fire.
 The employees are aware of the Emergency Preparedness and Response
procedures (EPRP) and immediately after noticing the fire, started
following procedure of the emergency preparedness and response.
 The first person to observe the fire was Stores Assistant. He started
shouting ‘Fire, Fire’, started using fire-fighting equipment and at the
same time, alerted the nearby people.
 The next person who was also a assistant pressed the siren push button.
 The key persons involved in EPRP participated in the procedure and
made fire-controlling act a success.

The following points are noticed during the procedure followed:

1. Immediately after hearing siren, key persons involved in EPRP reached


the emergency control center, from there to Emergency Area, and from
there attended duties by passing instructions.
2. Fire extinguisher of type ABC 150 Kg DCP is brought to the emergency
location, but it was not used.
3. Ambulance arrived at the location. The Operator affected by the fire was
taken to the first aid center. Stretchers were used in this process.
4. All employees present who were present in the warehouse and PB-I, PB-
II moved to the Gathering Area.
5. The type of fire extinguisher used was decided on the basis of the MSDS
(Material Safety Data Sheets) of the particular material that is present in
the drum.
6. Fire Trailer Pump (FTP) was started and the process to put off the fire
was initiated.
7. Foam carboys also arrived.
8. There was no communication problem.

 The mock drill was conducted on 20.06.07 at 10.30


 The location was Drum Shed near Effluent Treatment Plant (ETP).
 The material in the drum was n-Hexane
 This material leaked because of an accident.
 While transporting the n-Hexane drum, the drum fell on the opener, and
the drum was damaged
 The material in the drum leaked.
 The employees were aware of the Emergency Preparedness and
Response procedures (EPRP) and immediately after noticing the leak,
started following procedure of the EPRP.
 The superior present there informed the situation to the leak control
team.
 Leak control team arrived at the site.
 Contained the leak and transferred the material into another drum, and
removed the drum form the location.
 The material leaked in the drum shed was cleaned with water; the
wastewater was collected and sent to ETP.
 The key persons involved in EPRP participated in the procedure and
made containing the leak a success.

The following points were noticed during the procedure followed:


1. A rope was tied around the leak-effected area.
2. No Entry and Restricted Area boards were put.
3. Leak control team was wearing the required Personal Protective
Equipment (PPE).
4. Fire fighting equipment was brought to the accident location.
5. The material did not spill on any person.
6. Material Safety Data Sheet (M.S.D.S) of the leaked material was brought
to the location.
7. There was no communication problem.

 The material in the drum was Heptane
 This material leaked because of an accident.
 While transporting the Heptane drum, the drum fell on the opener, and
the drum was damaged
 The material in the drum leaked.
 The employees were aware of the Emergency Preparedness and
Response procedures (EPRP) and immediately after noticing the leak,
started following procedure of the EPRP.
 The superior present there informed the situation to the leak control
team.
 Leak control team arrived at the site.
 Contained the leak and transferred the material into another drum, and
removed the drum form the location.
