MEMORANDUM OF AGREEMENT

This Memorandum of agreement is entered into this 21st day of April 2022
by and between:

SAN MARIANO SUR ELEMENTARY SCHOOL, with School ID NO. 103692, a


public elementary school with office address at Barangay San Mariano Sur, San Guillermo,
Isabela, represented this Agreement by its School Head, RICHARD S. PICAZA, of legal age and
hereunder referred to as the SCHOOL;

-and-

BARANGAY OF SAN MARIANO SUR SAN GUILLERMO, ISABELA,


represented by YOLANDA R. BONGBONGA , who is the Brgy. Chairperson, entered in this
Agreement, Filipino, of legal age, hereunder referred to as the SERVICE PROVIDER.

WITNESSETH

Whereas the SCHOOL, is one among the schools entire San Guillermo to implement
Limited Face to Face and Modular Distance Learning, particularly within the Barangay of San
Mariano Sur San Guillermo, Isabela effective SY 2021-2022.

Whereas, the SCHOOL, based from the result of survey conducted to the parents of
school children, the Limited Face to Face and Modular Distance Learning is the most preferred
learning modality to be used by their children in acquiring education amidst this pandemic.

Whereas, the SCHOOL is currently on the verge of preparation and reproduction of
Learning Activity Sheets, Modules and other materials made/adopted by the teachers to respond
to the Limited Face to Face and Modular Distance Learning .

Whereas, due to limited face to face interaction between Teachers and Learners, these
LAS/Modules will be packed on weekly basis and to be delivered to the learners’ residences by
the “umalohokan” to be designated by the Brgy Officials.

Whereas, the Brgy Local Government Unit of San Mariano Sur, San Guillermo, Isabela,
headed by YOLANDA R. BONGBONGA, Brgy Captain, will designate a drop-off point and
delegate one “Barangay Kagawad per purok who will be in-charge of receiving and releasing
of LAS/Modules to the parents of the identified learners of the barangay. The schedule of
Limited Face to Face Learning will be posted too as soon as it is being approved

Whereas, the SCHOOL thru its designated courier shall deliver the packed
LAS/Modules directly to the drop-off point which will be received by the Purok Kagawad ,
distribute to the parents of learners every Monday and receives the LAS/Modules every Friday
from the parents, and release the said materials to the school- designated courier.

Now, therefore, for effective delivery of instruction this time of pandemic, and for and in
consideration of the foregoing premises, the PARTIES hereby agree as follows:

Description of the Project:

PROJECT “STEWARD LAMP”

STHRENGTHEN TEAMWORK AND RESPONSIBLE DELIVERY OF


LEARNING AMIDST THE MENACE OF PANDEMIC

I. OBJECTIVE OF THE PROJECT:

To safeguard the uninterrupted learning of children through the provisions and services
of the stakeholders in the delivery of learning modules to the learners’ while conducting the
Limited face to face learning delivery mode.

II. RESPONSIBILITIES OF THE PARTIES

A. Joint Responsibilities:

1. Both School and Municipal / Barangay Officials shall designate “courier” and Drop
Off Points per Purok. Brgy Tanod will serve as in-charge in COVID-19 Monitoring and
Health Check to individuals in front of the gate before entering the school premises.
2. Shall ensure that these modules shall reach the children to continue education and
follow the scheduled dates for learners in attending limited face to face learning.
3. The Barangay Kagawad and Brgy. Tanod shall perform their duties religiously for the
benefit of school children,

B. Responsibilities of the SERVICE PROVIDER

1. Create a joint working group with the school that will prepare the operationalization of
the extended limited face to face platform vis-à-vis the School Safety Assessment Tool
(SSAT) as stated in the DM #071, s. 2021
2. Provide supplies and materials needed as stated in the SSAT, and to strictly observe
health protocols among the teachers and learners in the school, such as

3. Form a joint steering committee to monitor the implementation of the BE-LCP during
the limited face to face modality.
4. Assign/designate Barangay Tanod on a daily basis who will assist the school in
maintaining order and uninterrupted flow of the Face to Face operations in school.
5. Assign/designate Barangay Health worker on a daily basis who will keep the triage
functional as stipulated in the SSAT.

C. Responsibility of the SCHOOL.

a. Managing School Operations


1. Ensure reproduction and delivery of free modules and LAS for the learners
2. Check, analyze and interpret the results of activity sheets and report regularly to the
parents and proper authorities about the learning development and engagement of
learners.
3. Adjust, align and reprogram appropriate activities subject to compliance to DepEd and
IATF memo and orders.

b. Managing School Safety Measures


1. Implement an Alternative Work Arrangement necessary to deliver quality basic
education in a safe learning environment to learners.
2. Orient teaching personnel on the AWA that will be implemented during the limited face
to face classes.
3. Establish mechanisms inside the classroom to ensure zero to minimal risk of COVID-
19 transmission of the learners.
4. Set-up clear and easy-to-understand signage to strengthen observance of health
protocols and protective measures.
5. Establish safe entrance and exit procedure for teachers, pupils, teaching and non-
teaching personnel, and school visitors.
6. Establish a contact tracing procedures/ tools for school-goers.
7. Mobilize the school COVID-19 DRRM team together with the School Safety
Officer and its team whom will take charge in ensuring effective implementation of
the school’s health and safety protocols that are in place and are observed during
the preparation and implementation of limited face to face classes.

8. Set up a proper sanitation and hygiene facility for school goers.
9. Ensure regular sanitation and disinfection of school facilities, furniture, and
equipment.
10. Ensure a proper disposal system on infectious / hazardous wastes, such as tissues,
and masks, and in non-contact receptacles.
11. Prepare an orientation session for learners, parents, guardians, teaching and non-
teaching personnel, external stakeholders, and LGU of the eligibility criteria for
participation, existing protocols, mechanisms, and procedures needed in the conduct
of the limited face-to-face classes.
12. Have a proactive COVID-19 local hotline/ help desk or any similar local
mechanisms that connects and coordinates to the hospital, testing facilities, and
LGUs.

13. Prepare a re-orientation session for school stakeholders on measures needed for the
reopening of the school in the event of resurgences

III. Addendum: The School shall inform and seek support from the BRGY Officials and
Municipal Local Government Unit on the conduct of Limited Face to face and Modular
Distance Learning among learners within the barangay on S.Y. 2021-2022.

IV. EFFECTIVITY

This agreement shall take effect within the school year 2021-2022.

V. PROGRAM COST

The project cost shall be determined after limited face to face for S.Y. 2021-2022.

Mataas Na Kahoy Elementary School together with its stakeholders is on the verge of
preparing its resources as it face the challenge of undergoing the limited face to face learning
modalities to be implemented this S.Y. 2021-2022. In view of this, may we be accommodated
for a dialog with you and your staff for information and dissemination on the implementation of
the said learning modalities this S.Y. 2021-2022 at your most convenient time.

Furthermore, may we seek assistance and support from your office, especially on the
conduct of Limited Face to Face learning delivery, and distribution/retrieval of our Self-Learning
Modules and Learning Activity Sheets to the learners, through any possible mobilization of our
dear Brgy. Officials and Tanod as “Service Provider or Umalohokan”.
