Successful in Changing in NIT Conditions
• Providing lighting in working area during execution of work, shall be responsibility of the
contractor.

• The equipment deployed by the contractors shall have all the safety features required by under
Reg. 216 of the CMR, 2017 and guidelines issued under it. (i.e. Gazettee notification no. G.S.R.
987(E) issued dated 01.10.2018 and the DGMS circular no. 06 of 2020; dated 27.02.2020).

• Gate pass shall be issued only to such equipments that are equipped with these features and after
examining their roadworthiness.

Fatal Accident at Dudhichua Mines due to toppling of tipper at 12:20pm on 6.4.2021

Brief:
A Fatal accident occurred at East BGR Dumping of Dudhichua
Mines when a scania tipper (no. SH-07) toppled on loaded Volvo
tipper (no. 83) at about 12:20pm on 6.4.2021.
This caused severe impact over the cabin resulting in fatal injury
to Shri Shree Nath Kewat (volvo driver no.-83)

Findings:
- The driver of tippers ignored the instructions of the
dump supervisor & did not follow SOP.
- The canopy did not hold load of the dullah of the

Fatal Accident at Bina OCP due to rollover of tanker at 1:15am on 21.4.2021

Brief:
While removing cross shaft from water tanker no.
4, standing on a mild down gradient, the water tanker
moved & rolled over Sri Mogili Eswar Rao, (21
yrs), mechanic helper, causing fatal injuries to him.

The tanker no. 4 which rolled The B/d tanker no. 1 at Filling point

Findings:
- Maintenance work being carried out without supervision & in Mine premises.
- Shortage of Spare parts
- Human failure in not using stopper & engaging the parking brake.

Fatal Accident due to runover by a tipper at Amlohri project at10:45pm on 20.10. 2021
The empty tipper no. 227
whose driver was overrun

The loaded tipper no. 254 toppled after running for about 20m Body of (Late) Sri Rajesh Prasad, at the spot of run-over

Brief:
The loaded volvo tipper (no. 254) while ascending the haul road got stalled in midway and started rolling. The tipper driver of another
empty tipper (no. 227), got overrun by this tipper.

Findings:
- After the volvo tipper stopped, none of the brakes functioned. The parking brake takes 20 sec. to get engaged that was enough to set a
running tipper to go uncontrolled.
- The driver of Volvo Tipper was operating in higher gear while climbing the gradient in order to overtake another tipper when the driver
was lowering the gear the tipper got stalled.

2. Status of Implementation of Road Map on Safety Culture & Climate
JHA based SOP for all jobs:
SOP for all jobs/operation have been prepared based on Risk Assessment process undertaken at the mines/projects
and number of SOPs updated in last C.Y. by all projects is as under:

3. Status of implementation on recommendations of High-Powered Committee.

 Model SOP framed & distributed to all the tipper operators. After few modifications amendments it will be
distributed to auxiliary equipment operators also.
 Skill based training to the drivers/operators engaged by HoE is carried out by a specialised trainer provided by
OEM.
 Board for competency evaluation has been formed in all the mines. Its effectiveness is to be enhanced.
 Few Changes in NIT document has been carried out:

4. Status of implementation of decision of 143rd CMDs meet of functions of ISO

 SMP – Could be reviewed only once.


 Safety Audit 2021-22 – Completed in two phases in all the mines from 12.2.2021 to 20.6.2021 & 4.8.2021 to
21.8.2021.
 SCAMP/Scientific Study – Scientific study of all mines as required under Reg. 106 has been completed.
 Bi-monthly meeting with Agents/POs of Mines by GM(S&R) –
 Monthly meeting with Manager of Mine & ASO – Monthly meeting with ASO is carried out regularly. Meeting
with Managers are held during mine visits/inspections.
 Guidelines issued by ISO for Contractual Manpower –
o Advisory on safe Operations at HoE dumps (dt. 31.5.2021)
o Distribution of SOP (dt. 28.7.2021, 5.8.2021 & 27.8.2021)
o Formation of Board for Competency evaluation (dt. 28.7.2021 & 28.8.2021)
 Inspection of Mines by ISO Officials – Is carried on monthly basis.
 Special safety drives undertaken – only 2 special safety drives were under taken:
o Special drive on Monsoon Preparedness.
o Special drive on Haul Road maintenance.

5. Status of Implementation of decisions of 148th CMDs Meet held on 25.11.2021

 10% increase of inspection by ISO Officials – The inspection by ISO officials are
carried on monthly basis than quarterly basis at NCL.

 Special Audit of HoE as per A-16 format - Could not be carried out in Nov & Dec
due to organisation of Annual Mines Safety week, holding of Bi-partite & Tri-partite
Area wise & that at Subsidiary level. The Audit of HoE as per A-16 format is being
carried out in this month’s ISO inspections.

 Health Check-up & Counselling for 50+ employees working in active areas – will be
undertaken in consultation with CMS.
