MEMORANDUM

FOR :  Chief of Police

SUBJECT :  Initial Investigation Report, Murder

DATE :  September 21, 2020

I AUTHORITY

1. Standard operating procedure of the police unit with reference to in

Murder section incident logbook, Entry No. 001-123 dated September 20,
2020.

II DETAILS

2. Upon receipt of the report in the police station, the investigators were
immediately dispatched by the Duty Desk Officer PSSgt. Restituto A.
Besas. The conducting investigation was to determine the alleged murder
of the victim identified as Aaron Jay B. Villapana.

3. At the scene, the initial investigation was spearheaded by PSMSgt. Donna

May Olaer. During the investigation, the police officers found out that the
suspect and victim argued because of Tong-its, a card game, when
suddenly the suspect ran away, came back holding a knife and stabbed
 the victim four times, as a result, the victim sustained stabbed wounds at
the left and right portion of his abdomen, one stabbed wound in his chest,
and one in his neck. The victim was dead when the police officers arrived.
The victim was brought to the Gov. Celistino Gallares Memorial Hospital
and was declared dead-on-arrival by Dr. Martin A. Villegas (Medico-Legal
Officer).

4. Other details of the incident were that one (1) witness, named Lhendon
Vergara Acedo, narrated how the incident transpired. The witness was
then invited to this station and attested his statements. Interviews were
conducted by this station with the victim’s neighbors and barangay officials
of the said barangay regarding the background of the victim and the
suspect. Interviews were conducted among family members, neighbors,
and barangay officials of both the victim and the suspect.

This case needs post mortem examination and will be referred to the Scene of
the Crime Operation (SOCO) for the proper disposition.

III RECOMMENDATION

5. It is respectfully recommended that further investigation be made to the

death of the victim in order to identify the cause of the victim’s death.
