AFFIDAVIT OF SERVICE

I, JO JAMES GAUDIA OBRA, legal age (34 years old), married, a Filipino
citizen, fish vendor, and a resident of Tammocalao, Bacnotan, La Union,
Philippines, after having been duly sworn to in accordance with law do
hereby depose and state that:

1. On ____ December 2021, I have caused to be served the

following pleading/ paper:

A copy of the Petition and its attachments in a

nullity of marriage entitled Ellen Flores Gregorio v.
Leonardo A. Gregorio, Jr., filed before the Regional
Trial Court, San Fernando City, La Union, by
PERSONAL SERVICE to the Office of the City
Prosecutor as evidenced by their stamp mark on
this Affidavit of Service; and

A copy of the Petition and its attachments in a

nullity of marriage entitled Ellen Flores Gregorio v.
Leonardo A. Gregorio, Jr., filed before the Regional
Trial Court, San Fernando City, La Union, by
REGISTERED MAIL to the Office of the Solicitor
General to their office address at 134 Amorsolo
St., Legaspi Village, 1229 Makati City considering
the distance, time constraints and quarantine
restrictions to effect personal service. This is
evidenced by the registry receipt hereto attached
for records purposes.

2. I execute this affidavit to attest to the truth of the foregoing and for
whatever legal purposes that it may serve.
