APPLICATION FOR ADVANCE FROM G.P.FUND

1. अंशदाता का िाम/Name of the Subscriber

5. आिेदि की तारीख को अंशदाता के खाते में कुल जमा निम्ि प्रकार हैः-

(ii) ददिांक से तक जमा अंशदाि

8. (क) ककस प्रयोजि के शलए अधिम चादहए

(a) Purpose for which the advance in required

(ख) नियम जजसके अंतगषत आिेदि अधिमान्य होता ह

(b) Rule under which the request is covered

(ग) यदद भिि निमाषण आदद प्रयोजिों के शलए अधिम
मांगा जाता ह तो निम्िशलखखत सच

(c) If advance is sought for House Building etc.

(i) प्लॉट की जगह तथा माप:-
Liquation and measurement of the plot:-

(ii) क्या यह प्लॉट पूणष स्िाशमत्ि में ह अथिा पट्टे पर ह:-

Whether plot is fee hold or on lease:-

(iii) निमाषण का िक्शा/Plan for construction

(iv) यदद फ्लट अथिा प्लॉट बबज्डंग सोसाइटी से खरीदा
जा रहा ह तो सोसाइटी का िाम, फ्लट का स्थाि
तथा उसका िाम आदद।
If the flat or plot being purchased from a H.B.
Society, the location and measurement etc.
(v) निमाषण की लागत
Cost of construction
(vi) यदद फ्लट दद्ली विकास प्राधिकरण अथिा ककसी
आिासीय बोडष आदद से खरीदा जाता ह तो स्थाि
पररमाप आदद का उ्लेख ककया जाए।
If the purchase of flat is from DDA or any housing
Board etc, the location dimension etc. may be given

(घ) यदद अधिम पररिार के सदस्यों के शशक्षण के शलए मांगा

गया ह तो निम्िशलखखत वििरण ददए जाएं
If advance is required for education of children,
Following details may be given

(i) पुत्र/पुत्री का िाम/Name of the son/daughters:

(ii) कक्षा तथा संस्थाि/कॉलेज जहां अध्ययि कर रहा ह

Whether a day scholar or a hostler:

(ड.) यदद अधिम पररिार के ककसी बीमार सदस्य के उपचार के शलए मांगा
गया हो तो निम्ि वििरण ददए जाएैः-
If advance is required for treatment of ailing family members,
Following details may be given:-
(i) रोगी सदस्य का िाम तथा उससे संबंि
Name of the patient and Relationship
(ii) अस्पताल/और्िालय/उस डॉक्टर का िाम जहां रोगी का उपचार हो रहा ह
Name of the hospital/dispensary. Doctor where the patient is undergoing treatment.
(iii) िह बदहरं ग/अंतरं ग रोगी ह
Whether outdoor/indoor patient
(iv) क्या प्रनतपूनतष स्िीकायष ह अथिा िहीं
Whether reimbursement available or not:
दटप्पणीैः 8(ग) से 8(ड.) के अिीि अधिम के मामले में ककसी दस्तािेजों के साक्ष्य अथिा प्राििाि
की आिश्यकता िहीं होगी।
In case of advance under 8(c) to 8(e), no certificate or documentary evidence would be required.

9. अधिम की समेककत ििराशश (मद 6 तथा 7) तथा माशसक ककश्तों की संख्या जजिमें
रु. अधिम राशश की ककश्तों का प्रस्ताि ह।
