Pursuant to the implementation of the SK Fairview Financial Aid Program, ___________________ _______,

I. NATURE
The Financial Assistance Project administered by the Sangguniang Kabataan of Fairview Barangay
is for the pursuance of Grade 9 to 12 students to continue their education despite grievances brought
about by the pandemic.

II. DURATION OF FINANCIAL ASSISTANCE


The Financial Assistance Project shall be for a duration of three months, from August to October of
the year 2020, unless earlier terminated due to failure of the Second Party to comply with the terms of
this Financial Assistance Agreement.

III. STUDY PLACEMENT


The party of the second half must be enrolled in any Department of Education recognized institution
offering junior and senior high school programs.

IV. ENJOYMENT OF ANY OTHER GOVERNMENT SUPPORT/ SCHOLARSHIP AWARD


The party of the Second Part may be allowed to enjoy a private scholarship administered by the
High School he/she is enrolled in simultaneously with this Financial Assistance Project, provided, that
the terms of the private scholarship do not interfere or are in conflict with his/her contractual obligations.

V. FINANCIAL AID PRIVELEDGES


The Party of the Second Part shall be entitled to receive financial assistance during the period of 3
months as stipulated in Section II of this Agreement, to be disbursed by the Party of the First Part,
subject to availability of funds for the purpose and to pertinent government accounting and auditing
rules to wit:
 For students enrolling as Grade 9 or 10 a tuition subsidy to be given at the start of the program
and later before its termination amounting to ₱1,900.00, and
 For students enrolling as Grade 11 or 12 a tuition subsidy to be given at the start of the program
and later before its termination amounting to ₱3,000.00.

VI. FINANCIAL AID OBLIGATIONS

A. Grade Requirement
The Party of the second Part shall maintain an adequate grade average with no failing grades in any
major or minor subject. Should the awardee incur any academic deficiency, his/her academic
performance shall be evaluated as such, the possibility of acquiring the same award would be uncertain
in the future.

B. Reports
The Party of the Second Part shall submit reports on a monthly basis regarding academic standing
such as but not limited to completed requirements, results of quizzes or exams, and grades for every
subject taken, as well as, reports on activities of the Sangguniang Kabataan which he/she participated
in.

C. Non-Compliance in the submission of report


The party of the Second Part shall be considered as a non-complying aid awardee if he/she stops
submitting the required reports for two consecutive months to the Sangguniang Kabataan Office. The
Financial privileges shall then be forfeited.

D. Physical Status and Moral Character


The Party of the Second Part shall maintain good physical condition and moral standing.

VII. RETURN SERVICE


The party of the Second Part shall render service to the Barangay by actively participating in other
activities or programs spearheaded by the later, for the minimum period that he/she has been given the
Financial Aid.

The SK-activities to be attended by the Second Party includes the ff:


a. Meetings / Online Seminars
b. Community Services
c. Information & Education Campaign

VIII. OBLIGATION OF THE PARENT/LEGAL GUARDIAN

I, __ ___________________ _____ the parent/legal guardian and representative


of the Party of the Second Part in this Scholarship Agreement hereby acknowledges, agrees and binds
himself/herself to:

1. Assume the civil liabilities that the Party of the Second Part may incur under the above written
Agreement as well as the civil liabilities which he/she may incur against other persons;

2. Shoulder the medical and other expenses in case of sickness, disability or death of the Party of
the Second Part;

3. Provide for other expenses of the Party of the Second Part which are not specifically mentioned
in the above Agreement;

4. Help and cooperate in the moral, spiritual and mental development of the Party of the Second
Part; and

5. Assume the obligation to pay whatever amount the Party of the Second Part shall be required to
refund in the event that he/she refuses to comply with the provisions of Section
