Instructions: Respond to each of the following questions with a complete sentence. You may receive if
a 100 if your answers show effort and quality. Please answer each prompt with at least one complete
paragraph.

Part 1: A big part of being successful now is knowing where you want to be in the future. As you plan for
your future this month list the 5 colleges you would like to attend below and why. If you do not plan on
attending a 4 year institution you can list technical colleges or training you plan on completing.
- University of Georgia: I would like to go to UGA because they have a sports medicine program
which is the career field, I would like to be in. I also love the campus and I like how it is close to
home, but not too close. I also like how the have a program where you can get two degree at
once and another where you can get internships, study abroad, and have research funded.
- University of Southern California: University of California has a sports medicine program and it is
close to my family members who live in California. I have heard they have great school spirit and
they have great events happening all the time. The weather is also great, which is a plus.
- Stanford: Stanford is close to my family and can keep me connected and they also have a sports
medicine program. They make time for students to have an active social life, have great sports
teams, great weather, and are known to have professors that are the best in their fields. Stanford
can also give me the opportunity to learn about entrepreneurship if I were to ever start my own
business.
- University of North Carolina: University of North Carolina has a high graduation rate and has a
great sports medicine program. Although the school is out of the state, I would only be a couple
hours away from family.
- Ohio State: Ohio state is very close to family and they also have a sports medicine program. But
if I want to change majors, explore more or double majors they allow me to do that as well. They
have great technology to help connect the students with each other and faculty and the
environment is very welcoming. The community has great spirit and pride. They are also is a
large city that is affordable and easy to get around, Columbus.

Part 2: Of the five colleges listed select one as your top choice. Write a one paragraph summary of what
this college has to offer you. Include information such as campus organizations or activities you are
interested in, your intended major and how this college will help you achieve your professional goals.
- The University of Georgia would have to be my top choice. For one thing I love the area it is in.
The whole city is basically all about the college and the campus is very close to stores and
restaurants if you need to get away. I also love how involved in sports they are. I love football
and might want to cheer in college, so I am glad that there is an opportunity for me to do that at
UGA. As I said before there is a program where you can receive you undergraduate’s degree as
well as another at the same time! This program is called double dawgs. I also love how students
tell you that professors want to get to know each of the students and that they have a positive
experience with the professors. I also enjoy the campus having WIFI wherever there is grass
because the study options are endless. The campus also has buses to move the students around
and I can get the zell miller and hope scholarship at this college. I will also still be close to home,
but not too close to where my mom will try to spy on me every weekend.
Part 3: How do you believe your time in Work Based Learning will help to better prepare you for your
future college and career plans?

- I think that WBL will help me gain a sort of professionalism that take experience to learn, rather
than just being told what to do or what it is like. I feel that is will teach me life skills that can set
me apart from the rest, but also help me find what I really enjoy doing so I can narrow down
what majors interest me most, and not have to switch majors mid-year.
