at AFP Medical Center, Victoriano Luna Road, Quezon City and/or any authorized
hospital/medical facility of AFP/PMA; and

e. I/we hold the AFP/PMA and/or any of its military and civilian personnel free and
harmless from any liability whatsoever for any death, injuries, loss or damaged caused by any
fortuitous events/ or force majeure, act of the public enemy, act or omission of the candidate
beyond the lawfully authorized scope of activities by AFP/PMA, and other analogous
circumstances;

2. I/we warrant under the law that I am/we are both lawfully authorized to sign this
documents.

3. I/we undertake to indemnify the Armed Forces of the Philippines and/or the Philippine
Military Academy (PMA) for whatever damage incurred should any entries that I/We provided in
this document be found falsified;

4. I/we executed this document to attest to the truth of all the foregoing facts, and for any
other lawful intent and purpose it may serve; and
