JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, MARCELINO N. MANLANGIT and PAZ B. MANLANGIT, both of legal age,

Filipino citizens, married to each other and residents of Lot 1. Blk.7, Housing Phase III, Purok 8,
Tiptip, Tagbilaran City, Bohol, after being duly sworn in accordance with law, hereby depose
and state that:

1. We personally know the deceased EVELYN D. MAGHUYOP, who was a resident

of Lot 7, Block 6, Uptown Phase III, Titptip Dist., Tagbilaran City, Bohol, being
close friends of the family and neighbors;

2. This affidavit is executed to attest to the truthfulness of EVELYN D.

MAGHUYOP’s true identity;

3. EVELYN D. MAGHUYOP is the daughter of Perfecto DUMAPIAS and Purificacion

JOHNSON;

4. EVELYN D. MAGHUYOP has a maiden name of EVELYN JOHNSON

DUMAPIAS, and the same has been her TRUE and CORRECT maiden name;

5. We know for a fact and can vouch that the person of “EVELYN J. DUMAPIAS” as
found on the SSS records and the Marriage Contract, and “EVELYN DUMAPIAS
MAGHUYOP” appearing on the Death Certificate issued by the Office of the Civil
Registrar General, Tagbilaran City who died on January 11, 2021, refer to ONE and
THE SAME person;

6. We further attest that the TRUE and CORRECT birth name of Evelyn D.
Maghuyop is EVELYN J. DUMAPIAS and not ABELIN DOMPAIS as found on
the Certificate of Birth issued by the Office of the Civil Registrar Loboc, Bohol. The
correction was annotated in the same Certificate of Birth, pursuant to the Decision
rendered by the OIC-MCR Samuela L. Fuertes dated December 16, 2014;

7. Furthermore, we likewise attest that the father’s name of Evelyn D. Maghuyop

appearing on the Certificate of Birth, “PERFECTO DOMAPIAS” and
“PERFECTO DUMAPIAS” as appearing in the Marriage Contract and the Death
Certificate of Evelyn Maghuyop, refer to one and the same person. Moreover, the
TRUE and CORRECT father’s name of Evelyn D. Maghuyop is “PERFECTO
DUMAPIAS” as found on the annotation made on the Certificate of Live Birth of the
former;

8. In addition, with regards to the mother’s name of Evelyn Maghuyop, we further attest
that “PURIFICACION YANSON” as found on the Certificate of Birth and the
Marriage Certificate of Evelyn Maghuyop, and “PURIFICACION JOHNSON” as
appearing in the Death Certificate of Evelyn Maghuyop, refer to one and the same
person. Also, the TRUE and CORRECT mother’s name of Evelyn D. Maghuyop is
“PURIFICACION JOHNSON” as found in the annotation of the same Certificate of
Birth of Evelyn Maghuyop;

9. We further attest that the foregoing discrepancies are due to the inadvertence and
oversight of the Officers in charge of the different agencies leading to various
typographical error. Moreover, the discrepancies found on the Certificate of Live
Birth of Evelyn D. Maghuyop have already been corrected pursuant to a decision

 rendered by OIC-MCR Samuela L. Fuertes dated December 16, 2014. The said
corrections had already been annotated in the same Certificate of Live Birth;

10. We are not related by affinity or consanguinity to EVELYN DUMAPIAS

MAGHUYOP; and

11. This affidavit is executed to attest to the truth of the foregoing facts, inform the
proper agency and for all legal intents and purposes it may serve.
