REASON FOR OVERPRESSURE / NO OVERPRESSURE

S.NO. CONTINGENCY APPLICABLE
(For detailed justification refer to notes)
1a. CLOSED (BLOCKED) OUTLET Yes Mist Eliminator blockage or Closure of UV -0012 will cause Closed vapour Outlet scenario.
Vapour

1b. CLOSED (BLOCKED) OUTLET Yes Liquid collected in the KOD is rare and very trivial. Operator has adequate response time to take
Liquid action. Hence this case is not credible.

1c. REVERSE FLOW Yes Vapor outlet goes to LP fuel gas drum and Distribution header. Both are operating at lesser pressure
than relieving pressure. Hence this case is not credible.

2. COOLING WATER FAILURE No This system does not have any cooling water users. Hence Case is not credible.

3. REFLUX OR PUMPAROUND No This system does not have any reflux or pump around users. Hence Case is not credible.
FAILURE
4. SIDESTREAM REFLUX FAILURE No This system does not have any reflux or pump around users. Hence Case is not credible.
5. FAILURE OF ABSORBENT MEDIUM No This system does not have any absorbent medium. Hence Case is not credible.

6. ACCUMULATION OF No This system does not have any condensers. Hence Case is not credible.
NON-CONDENSIBLES
7. ENTRY OF VOLATILE MATERIAL No This system does not have Hot and cold liquids. Hence Case is not credible.
8. LIQUID OVERFILL Yes Blocked liquid outlet can cause Liquid Overfill. However case is not credible. Refer Case 1b for
Process or Storage/Surge Vessels details.
9. CONTROL VALVE FAILURE Yes 1. Failure of HS-0004: Failure of HS-0004 during startup can open all Control valves (PV-0004 A-D)
Open or Closed connected to it or it can close all the Control valves. Closure of control valves will not overpressurize
the system. However opening all control valves will cause overpressurization. Case is credible.
2. Failure of HS-0005: Failure of HS-0005 can open all Control valves (PV-0005 A-F) connected to it
or it can close all the Control valves. Closure of control valves will not overpressurize the system.
However opening all control valves will cause overpressurization. Case is credible.
3. Individual control valve failures are not significant. Hence not considered for calculation.

10. ABNORMAL PROCESS HEAT INPUT Yes Failure of HS-0001 can cause both 1540E-001A/B to operate. Case is credible.

11. EXCHANGER TUBE RUPTURE No There is no Shell and tube Heat exchanger. Hence case is not applicable.

12. INTERNAL EXPLOSION No There is no oxygen inside the system. Hence Case is not applicable.
(Excluding Detonation)
13. CHEMICAL REACTION No There is no chemical reaction inside the system. Hence Case is not applicable.
14. THERMAL EXPANSION No System is gas filled system. Hence Case is not applicable.

15a. FIRE Yes Fuel Gas KOD is assumed to be located above 1.5m from grade Which is less than fire Elevation.
Vessel with Partial Liquid Inventory Hence Case is credible.

15b. FIRE Yes Fuel Gas KOD is assumed to be located above 1.5m from grade Which is less than fore Elevation.
Vessel Filled with Gas (No Liquid) Hence Case is credible.
15c. FIRE No System is gas filled system. Hence Case is not applicable.
Vessel Filled with Liquid

16. POWER FAILURE Yes Power failure can cause Heater to trip. This will not cause overpressurization. Hence Case is not
credible.

17. INSTRUMENT AIR FAILURE Yes All control valves and on /off valves in this system is Failure closed. Hence no overpressurization.
Case is not credible.

18. STEAM FAILURE No Steam is not used in this system. Hence not applicable.

19. FUEL GAS FAILURE Yes Fuel gas failure will not overpressurize the system. Hence this case is not Credible.
20. INERT GAS FAILURE Yes Inert gas is used for purging. Failure of Inert gas during purging will not overpressurize the system.
Hence case is not credible.

21. NITROGEN PURGING ISOLATION Yes Maximum nitrogen operating pressure is 7.5 barg which is lesser than relieving pressure. Hence case
VALVE FULLY OPEN is not credible.

Basis of Calculation
Scenario 9.Control Valve FailureOpen Or Closed
Multiple control valves are connected to Single selector switch HS-0004. Selector switch allows user to select particular set of control valves
for pressure controller set point. In case this control switch failure, there is a possibility of control signal routing to all control valves. During
failure it is assumed controller generates 100% opening of control valves. This means both Main valve and bypass valve opens to its fully
open position.

Governing Case Identification:

HP Fuel gas KOD recieves fuel gas from three sources and five different operating modes are available in Fuel gas KOD. Each case
sheall be Evaluated and control valves shall be sized for governing case before proceeding into calculation. Highest volumetric flow
case shall be considered as governing case for Control valve sizing.

BASIS OF CALCULATION Pressure Unit bar
To calculate the relief flow when an upstream control valve fails open. Any bypass valve is assumed to be 100% open. Credit may be taken for the
normal outlet flow. The required Cv for the control valve is calculated based on normal upstream and downstream operating conditions. The
selected (design) Cv for the control valve (and by-pass valve, if provided) is estimated and used to determine the maximum flow through the fully-
open valve(s) at relieving conditions.

Controller Failure: HS-0005 Failure
Failure of HS-0005: Failure of HS-0005 during startup can open all Control valves (PV-0005 A-F) connected to it or it can close all the Control valves. Closure of
control valves will not overpressurize the system. However opening all control valves will cause overpressurization. Failure of PV-0005 A-B will not cause
overpressurization due to less operating pressure than relieving pressure.

Scenario 9.Control Valve FailureOpen Or Closed
Multiple control valves are connected to Single selector switch HS-0005.
