Rubrics. Students who failed will undergo RESIT which is scheduled as per
quality standards of the institution.
Students who FAILED will undergo RESIT which is scheduled accordingly. A
Re-Sit student, who fails in the same competence 3 times, shall be allowed to take
RESITS until passing scores are met.
Final Rating The student should get a minimum mark of THREE (3) USING THE
RUBRICS to pass the assessment.
DECISION and • If your score is 75 points and above, then you are COMPETENT. You

GUIDELINES:
1. First, you need to download this file. Afterward, you need to print the form.
(NOTE: Print page 5 to answer your assessment. ONLY Page 5 should be sent in the LMS.);
2. On Page 5, once printed, fill out the form by indicating your NAME, COURSE PROGRAM, DATE
and put a check mark on the box that corresponds the number of times that you will take this COA;
3. Your answer must be handwritten and in essay form.
(NOTE: Manuscript or Cursive Handwriting is allowed to use.);
4. Please do not share your answers with other students since I am going to check your COA one by one.
In case, there are answers that are literally the same as other students and/or there are files that were
already submitted by your classmates, then this will be result in a failing grade (Resit).
(NOTE: Again, make your own COA and do not rely on your classmates. Please do not copy- and-
paste your answers from the internet/modules and the idea must be coming from your perspective.;
ALWAYS OBSERVE ACADEMIC HONESTY.
5. If you are done, take a clear picture of your COA ANSWER SHEET, at the same time picture of
yourself while answering this assessment. (It must be before and after selfie pictures.)
(NOTE: Make sure that is readable. Since your output is a picture file, this is preferably in JPEG
PICTURE FILE format so that I can zoom it out.);
6. Once done, upload/ save your output in your Google Drive (create your Personal Google drive if you
don’t have one.) Get a copy the shareable link of your output and paste it in the Online text Box to
submit your activity.
(NOTE: Do not forget to check the share permission in your Google Drive).
7. Next, write also on the Online text box the number/frequency of your COA attempt beside your name
and the shareable link of your COA.

8. Submit this activity on or before the deadline.
(NOTE: The PhilCST LMS will not accept any activity after the deadline).
9. Read the rubrics in Page 4.
(NOTE: The rubrics is a scoring tool that lists the criteria for your Course Outcome Assessment.
Rubrics list what needs to be included in order to receive a certain score or grade which allows you to
evaluate your work before submitting your COA.
