AFFIDAVIT OF EXPLANATION

I, CAMILO III CORNELIO ANDAL, of legal age, Filipino,

married, and presently residing at Blk 4 Lot 76 Vanda St. So
Garden Hms Salitran III, City of Dasmariñas, Cavite, after having
been duly sworn to in accordance with law, depose and state that:

1. I am the holder of Passport No. EC7969163 issued at DFA

Manila;

2. My passport is due to be renewed on May 17, 2022.

However, as of now, I am unable to locate this old passport;

3. That diligent search of said document proved futile and

unavailing;

4. I then explained to DFA Manila the circumstances

surrounding my lost-passport and on my Passport
Application Form, and they told me to prepare an affidavit
to such effect – thus this affidavit;

5. That everything was done in good faith and of honest belief;

6. That the application for new passport done in DFA Manila

on May 17, 2022 is not for the purpose of fraud and
perjury;

7. That I attest to the truthfulness of the contents of this

affidavit of explanation.

IN WITNESS WHEREOF, I have hereunto set my hand this
