constitute and appoint, EREBERTA HAYAG ARIBAL, Filipino, of legal age, married,
and with address at One Tanza Subd., Brgy. Julugan VI, Tanza, Cavite, Philippines,
Philippines, as my true and lawful attorney-in-fact for me and in my name, place and
stead, to do the following acts and deeds, appear, to wit:

1. To represent me and in my behalf in the processing of my correction of name

in the Bureau of Internal Revenue (BIR);

2. To execute, make and sign any/all papers or documents necessary or

indispensable in connection with the foregoing purposes;
