SCENE RESPONSE-High Profile Case

PRELIMINARY CASE INFO: Dump truck vs Pedestrian, no charges pending or anticipated. Male decedent
was reportedly witnessed waving cars down on the shoulder of the Interstate 595 westbound, then struck by
a dump truck, then either struck or ran over by an SUV. No suspicions of foul play.

INITIAL INFORMATION RECEIVED: On 4/9/2022 at 0815 hrs, M.E. Office Chief Investigator Thomas
Steinkamp called and advised me to generate a case number for a male pedestrian struck by a car on
I-595, then respond quickly to the scene. More information will be provided on scene.

When I arrived on scene, I took overall photos, then quickly set up the body shield. Then I received the
following information.

Per FHP Sergeant Kudla, witnesses began calling dispatch at 0633 hrs for a pedestrian walking in lanes of
traffic on the I-595 westbound before I-95 northbound exit. One female witness stated she seen a black
male dressed in all black on the right shoulder waving people down. She pulled to the right, parked, and
went to get out to see if he needed assistance. By that time, he had already been struck by a dump truck.
Dump truck driver states he was driving along in the darkness, and all the sudden a male appeared standing
in his travel lane, which at that time was the center lane. He struck decedent, who then traveled an
undetermined amount of feet, and came to final rest in the left travel lane, at which point a male driving an
SUV swerved to avoid hitting him, either running him over or striking him (this is still under investigation),
and almost striking the center median wall. Both dump truck and SUV pulled over to the right of I-595 and
awaited LE. Fire Rescue responded and pronounced him deceased at 0647 hrs with no medical
interventions performed.

Decedent was identified as 24-year-old black male Dwayne Elliott Haskins Jr, a quarterback for the
Pittsburgh Steelers, identified using the Pennsylvania Driver License found in his wallet on his person.

Addendum:
On April 10, 2022, I called Sergeant John Baker with FHP to gather more information uncovered during the
course of their investigation. They located a vehicle belonging to the decedent. A female companion was
inside vehicle. They had run out of gas on the side of the I-595. Decedent left on foot to locate a gas station
at or around 0615 hrs. Female was with him from 1900 hrs 4/8/2022 until he left looking for gas. She
remained in car until FHP found her. It appears to be accident. He will follow up with more information when
available. He is sending other investigators out to area today to attempt to locate the other shoe and cell
phone. He did not disclose the nature of this female and decedent's relationship. Information relayed to Dr.
Darin Trelka and Dr. Rebecca MacDougall.

FAMILY/WITNESS CONTACT: On April 9, 2022, I spoke with Head of Security for the Steelers, Jack
Kearney, who asked that we not call wife today as she is devastated and does not wish to speak to anyone.
Per Jack, decedent had no history of mental health issues, and no history of suicidal ideations or attempts.
He and the wife had a solid relationship with no recent fights or issues. Decedent was training with his
teammates all day, then they all went to dinner. After dinner, decedent and a friend/cousin named Joey went
to a club, possibly in Miami. They drank heavily and at some point, they got Page

teammates all day, then they all went to dinner. After dinner, decedent and a friend/cousin named Joey went
to a club, possibly in Miami. They drank heavily and at some point, they got into a fight, separating.
Decedent does not use illicit drugs other than marijuana. He does drink ETOH, heavily sometimes. He
would never take any unknown substances. He did not smoke tobacco. He had no known medical history.
He did not take medications daily.

SCENE OBSERVATIONS: Decedent was lying supine on the roadway in the left travel lane covered by a
yellow tarp. It appears he was originally lying prone or on his side, and he was rolled by Fire Rescue to
attempt life saving measures. One single high-top sneaker was approximately twenty-five feet in roadway in
front of body. No other shoe located on scene. No socks located on scene. No vehicle located in vicinity. No
cell phone located on scene.

BODY OBSERVATIONS: Algor was cold. Rigor and livor were not noted. Head trauma and leg trauma were
noted.

PERSONAL EFFECTS/DISPOSITION: Decedent was fully clothed in a black shirt, black pants, and black
underwear. A white and black women’s jacket was tucked under his left side. A white metal stud earring with
a clear stone and white metal back was removed from his right earlobe. Two white metal chain necklaces,
one with a cross pendant and multiple clear stones, and one with a number three pendant with multiple clear
stones were removed from around his neck. A hotel key card was removed from his right front pants pocket.
His wallet was lying near the body when I arrived. A white metal ring with multiple clear stones was removed
from his left ring finger. Two beaded bracelets and two belt like bracelets were removed from his right wrist.
One green, tan, and black satchel was removed from around his neck and left arm. Inside the satchel was a
blue belt type bracelet, a pair of eyeglasses, a small black container containing residue of unknown
substance, a lid to a container or spray, and a container of Icebreaker mints. All jewelry and personal effects
were handed over to FHP Trooper Kudla on scene. The shoe and the women’s jacket were brought into the
ME with the body.
