VIDEOKE RENTAL AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This VIDEOKE RENTAL AGREEMENT (“AGREEMENT”) is made and executed by and between:

NENAVIC D. PALENCIA (“LESSOR”), of legal age, married, Filipino, with residential address at
B37 Lot 3 4th Road Hermilinda Homes Subd., Brgy. Mansilingan, Bacolod City;
-and-
________________________ (“LESSEE”), of legal age, ________________________, Filipino,
with residential address at
______________________________________________________________________.

By renting from the LESSOR, LESSEE agrees to the following:

LESSEE acknowledges that he/she received all equipment (Videoke machine, Television,
Microphone, etc) are in good operating condition and agrees to maintain such equipment in
good condition.

If during the rental, equipment is lost, stolen, or damaged by fire, theft, water, or act of God, and
any cause whatsoever, whether or not fault of the LESSEE, the LESSEE shall compensate the
LESSOR for the full replacement value of the equipment. If the equipment in question is no
longer replaceable by the same make and model, the LESSEE agrees to compensate the LESSOR
for a model of equal in value, features, and performance to the lost, stolen, or damaged
equipment.
