Instructions
1. Please carry the registered mobile phone and the requisite documents, including appointment slip, the Photo ID card
used for registration while visiting the Enrollment center, for verification at the time of verification.
2. Please arrive at the Enrollment center only 15 minutes before the scheduled time to avoid overcrowding.
3. Please follow COVID appropriate behaviour at the vaccination centers.
a) Wear a mask.
b) Maintain a distance of at least 2 meters from others.
c) Follow the queue system maintained at the Enrollment centers
4. If applicant’s original Marksheets/Certificates are submitted at the organization where he /she is employed and unable to
present the same at the UPPC for verification then applicant must get an NOC from the concerned organization stating the
above condition with the Signature of Authorized Person and Organization ’s Stamp; and present it to UPPC at the time of
verification. Along with, carry the photocopies of required documents too.
