Incident : LTI of a contractor employee

Injury : Right leg Knee – Blood clotting (Pre-patellar Haematoma)

What happened:
Injured Person (IP) was moving towards plate storage area where he tripped over a projected part (approx.
10 mm) on a plate, kept over the cable trench and fell on his knee. His right knee got injured. Initially no pain
was experienced but later IP experienced pain and observed swelling. IP was taken to TGS first-aid from
where he was referred to Tata Main Hospital. IP was diagnosed with blood clotting (Pre-patellar Haematoma).
No bone injury diagnosed.

Preliminary Findings:
1. IP was wearing required PPEs.
2. Plate was having multiple projections which led to tripping hazard.
3. IP was inattentive.

Immediate actions taken:


1. IP was taken to TGS First Aid and then moved to TMH emergency unit.
2. All operations stopped and area was cordoned off.
3. PIR (IN00139686) logged in Ensafe.

Recommendations:

1. Existing damaged trench cover to be replaced.


2. All trench covers to be reviewed for tripping hazard.
3. Training and awareness session on Slip/Trip/Fall.
