It is mandatory to have a copy o this authorization on site during the activities, available on device or paper
To start any activities you must activate the work permit on GIMMI and inform the TCO
You have to observe the working area and the safety notes as indicated on the work permit
The delimitation of the working area is mandatory

Follow the instruction to apply the short circuit devices if needed (for the Switch Operator)

The shutdown is not required for this activity; further coordination is required with CWG if it is required to open the raised floor tile. • The contractor
requesting PTW has been informed that equipment and connecting cables are energized and assumes all the risk while working nearby
Safety notes

FOREMAN is responsible for clean up after work in the above-mentioned area.

If any leftovers are in the workarea (garbage, debris, tools, equipment, storage, cables, etc.) before he starts his work in the area, he must
inform the TCO otherwise he will be kept responsible for the leftovers.
It is mandatory to signal the real working area inside the chainage assigned to work activity ATTENTION: Loco Diesel will be traversing via
your work area. When the passage is required clear the working and all personnel must be in a safe position.
Follow HSE procedure and segregate the working area
Rigid protection must be applied on any exposed EWG equipment before any work at the area above that equipment is performed. All
necessary protective measured should be taken when working near technical equipment and they should be protected from flames and/or
sparks.
This work permit must be opened and closed with the TCO Operative Team (phone number 054 7295391) at the end and at the beginning of
the activities on a daily basis. Without the opening the work permit will not be considered valid.
BEFORE STARTING ANY ACTIVITY IN ELECTRICAL ZONE YARD AREA AND VIADUCT OR ANY ACCESS TO THIS AREA , ALL
COMPANIES SHOULD HAVE A VALID WORK PERMIT CALL TCO TO OPEN/CLOSE PTW , EXCHANGE POWER ON/OFF TEMPLATE
AND APPLY LOTO PROCEDURES. ANY VIOLATION WILL BE FOLLOWED BY SERIOUS ACTIONS
COVID-19 PRECAUTIONS: 1) 2 Meter of social distancing must be ensured during the activity. 2) It is compulsory to wear a mask to avoid
droplets and wear sterilized gloves during the activity. 3) It is highly suggested to wash your hands often with sanitizer during the activity. 4) If
there are any symptoms of COVID-19 call immediately on EMERGENCY NUMBER 0114516700 5) It is foreseen to have handshakes. 6) As
much as possible ensure to sanitize the equipment and assets used on site.
INFRINGEMENT FORBIDDEN: It is strictly forbidden to perform any permanent/temporary installation that may affect train/vehicle
movement. The activity supervisor at the end of the activity must ensure that no kinetic envelope infringement has been performed. The
closure of the Permit to Work at the end of the working shift has to be considered as a clearance declaration of the area. In case of potential
obstacles to the vehicle, the movement must be notified in advance to TCO otherwise it will be considered as a serious violation. The
Supervisor will be considered responsible for damage/delay due to any unauthorized and/not communicated installation
"* TRACK ACTIVITY (FUNCTIONAL STRECTH): 1) Storage of material is authorized keeping in between Tracks with considerable distance
from 3rd rail and material should be in tight condition and No physical infringement on 3rd rail. Run rail clear of any Metallic contact; 2) One
track should be completely free from any obstruction; 3) Contractor requesting PTW will provide a daily guarantee to certify line section
clearance and exchange Track readiness with TCO central Operator; 4) The subcontractor will provide the Track with housekeeping upon
completion of works; 5) The light material that can fly with high wind must be stored in a tight condition. 6) It is mandatory to install a position
flag (day shift)/light (night shift) 100 mt before and after the working area (must be removed at end of the working shift; Manual Trolley
Prescriptions: - It is forbidden to use manual trolley on the slopes; - Each Rolling Stock MUST be equipped with at least 2 chocks; - It is
forbidden to by-pass manual trolley safety devices; - Personnel involved in Manual Trolley movements must be properly trained; - All the
Manual Trolleys should be properly mainteined and certified; 7) LOTO PROCEDURE MUST BE APPLIED AND FOLLOWED BEFORE
GETTING ACCESS ON TRACK. THE WORK CAN START AFTER TCO CONFIRMATION ONLY WARNING : THE RED MEDIUM
VOLTAGE CABLE IS ENERGIZED IN CABLE TRAY IN VIADUCT , DON'T DRILL , CUT AND DIG NEAR THIS CABLE"

CLOSURE
Work permit closure

Inform the TCO before starting the activities to close the work permit and after the activities, in order to receive instructions about particular
conditions
The work permit is closed by the Foreman online and informing the TCO
The delimitation of the working area must be removed before closing the work permit
The area must be clear of all equipment, tools and people involved. The area must be in the same condition as before starting the activities
Ask the TCO for instructions to park the motor bogie / trolley.
