AFFIDAVIT OF CONSENT

We, MERLYN GARCIA, ALDRIN BUGAYONG AND ARMANDO GELACIO

SR., of legal ages, Filipinos and residents of Rizal, Moncada, Tarlac, after
having been duly sworn to in accordance with law, depose and state:

1. That we are the neighbors of CRISTY PAULINE GARCIA FLORES;

2. That CRISTY PAULINE GARCIA FLORES, is the registered owner of a

private resort with business name “RF Private Resort” located at 44
Rizal Moncada, Tarlac;

3. That CRISTY PAULINE GARCIA FLORES, is applying for a Business

Permit so she could operate her abovesaid private resort;

4. That we do hereby signify our written consent, freely, willingly and

voluntarily to allow CRISTY PAULINE GARCIA FLORES to operate her
business within the neighborhood, subject to the following conditions, to
wit:

a. That the said business should ensure that no noise and

other disturbances within the premises at all times;
b. That they will ensure safety and security within the
neighborhood; and
c. That they will create a reliable and effective drainage plan to
avoid flood and other issues that will affect the
neighborhood;

5. That I am executing this affidavit to attest the truth of the foregoing facts
as to our undertaking to give consent to CRISTY PAULINE GARCIA
FLORES to support her application for Building Permit.
