AFFIDAVIT OF CANCELLATION OF ADVERSE CLAIM

I, MA. VICTORIA M. DE LEON, of legal age, Filipino, married, with residence address at #144
Aquino Street Annafunan East, Tuguegarao City, after having been duly sworn to in accordance with
law, hereby depose and state, that:

1. I am the adverse claimant in a Notice of Adverse Claim annotated at the back of Transfer
Certificate of Title No. T-159261, in the name of Sps. Ricarte P. Cambia and Elvira F. Cambia and
issued by the Register of Deed of Tuguegarao City. The land covered by Transfer Certificate of Title
No. T-159261 consists of Three Hundred Square Meters, more of less.

2. My Notice of Adverse Claim was annotated as Entry No. 2014007363 at the back of Transfer
of Certificate of Title No. T-159261 and it reads as follows:

“NOTICE OF ADVERSE CLAIM: EXECUTED BY MA. VICTORIA M. DE LEON, CLAIMING

AMONG OTHERS THAT SHE IS A VENDEE OF THE SAID PROPERTY, IN ACCORDANCE WITH
DOC. NO. 69, PAGE NO. 15, BOOK NO. CCV, SERIES OF 2014, OF NOTARY PUBLIC OF
TUGUEGARAO CITY, CAGAYAN BY ATTY. CARLOS P. RIVERA, DATED OCTOBER 24, 2014.

3. I hereby request the Register of Deeds of Tuguegarao City to cancel or cause the cancellation
of the annotation of the Notice of Adverse Claim annotated on TCT NO. T-159261.

4. I am executing this affidavit to attest to the truth and veracity of all the foregoing and for all
other legal intents and purposes.
