MONTHLY CERTIFICATE FOR THE MONTH OF: May 2022

1. We confirm that the same Operator (Name of Operator and Operator ID) is working
in the AEC who has been approved and onboarded for the Branch and no other
person was allowed to work as AEC operator at any point of time.
2. We confirm that the Aadhaar Kit is kept in the Branch Custody overnight and is not
allowed to be taken outside the branch premises or be used beyond business hours
at any point of time. Further, it is delivered back to the onboarded operator in the
beginning of the day in the presence of the verifier.
3. We confirm that the Bank’s Official is performing the role of ‘Verifier’ and on ‘No
occasion’ during the month, any enrolment/update has been carried out without the
verification activities carried out by the designated verifier.
4. We confirm that the AEC area is covered under CCTV surveillance to keep a watch
on the activities of the operator.
5. We confirm that we are keeping a watch on the activities of outsourced operator to
guard against any instance of malpractice/charging extra money from the residents
through daily scrutiny of the “feed” of CCTV camera and through interaction with the
residents at the time of verification of their documents by the verifier.
6. We confirm that we are also checking regularly that the operator is not using dummy
fingerprints for rendering Aadhaar Services.
7. We confirm that no instance of malpractice or corruption or overcharging by the
operator was noticed by us, or was brought to our notice by any of the residents
during the month, except the following:

8. We confirm that the Aadhaar Enrolment/Update documents are not retained at the
branch and are invariably returned to the residents.
9. We confirm that rate list, as prescribed by UIDAI, for Aadhaar Enrolment/Updates is
prominently displayed in the branch.
10. We confirm that SOP relating to the role of Verifier and Operator circularized vide
Circular no NBG/FIMF-AADHAAR/9/2017-18 dated 01.03.2018 is being meticulously
complied with.
11. We confirm that the fee for Aadhaar enrolment/update is collected in the BGL
Account- “4899024-COMMISION ON AADHAAR SERVICES” and is tallied at the
end of day with the number of Aadhar updates done by the outsourced operator.

We are aware that Bank will initiate appropriate action if the above
confirmation being given by us is found to be incorrect at a later date.
