This is with regard to your Application for DAR Clearance involving Lot No. 162-H, Bsd-
06-001237, covered by Transfer Certificate of Title No. EP-5863, registered in the name
of Juan A. Jornadal, with an area of 0.0790 ha., more or less, situated at Brgy. Ungka II,
Pavia, Iloilo.

Please be informed that upon proper perusal of documents submitted to this Office, it
was found out that you were not able to submit the following:

1. Certified True Copy of Buyer’s Income Tax Return (BIR Form 1701);
2. Affidavit of No Pending Case; and
3. Special Power of Attorney

The aforementioned documents are essential requirements needed for the approval of
your Application.

In this connection, please submit the required documents within ten (10) days from
receipt hereof. Failure to submit the same is a ground for dismissal of your application.
