I, the undersigned researcher from the Grade 11 GAS G of the Pampanga High School, is
currently conducting a study titled “Holy Eucharist from the Lens of Select Public Elementary
School Students: An Exploration”. The said undertaking will inquire into the perceptions of
select public elementary school students on the significance of the Holy Eucharist in their lives.
This endeavor will be pursued in the hope of proposing a catechetical formation guide on the
discussion of the Holy Eucharist for public elementary school students in the Archdiocese of San
Fernando, Pampanga.

In view of this, I would like to respectfully request from you to please help me in this study by
being my validator of my data gathering instrument which is a semi-structured interview
guide. I believe that your expert opinion, suggestions and recommendations regarding this matter
