SUBJECT: Investigation Report on Shooting Incident which resulted the


death of one DARUIS MALABANAN

3. To identify the perpetrator/s and to file appropriate charges in court if warranted by


evidence.
III FACTS IF THE CASE
4. At about 10 am of April 16, 2016 our office receive a report through a mobile call by a
concern citizen identified as Margie Pedrosa y Galanza, 28 years of age, a tricycle
driver, resident of Canlubang St. Brgy. Mayapa, Calamba City, Laguna. That she witnessed
a shooting incident in Calubang St. Brgy. Mayapa, Calamba City, Laguna. respond of the
incident led by PINSP JIMALYN ABANCIA PAETE, DCOP, immediately went to reported are for
verification, Upon arrival at the same scene, first responder saw a body of a man covered with
blood while lying on the floor, identified as Daruis Malabanan y Mercado, 37 years old, single,
former security guard and a resident of Canlubang St. Brgy. Mayapa, Calamba, City, Laguna,
which after conducting examination of the crime scene was declared dead.
5. According to the witness the victim who was identified as Daruis Malabanan y
Mercado, was leisurely walking along the Canlumabng St. Brgy. Mayapa, Calamba City beside
at Itlog ni Kuya Food house when suddenly he was shot by an unidentified suspect who
immediately fled from the crime scene wearing a black jacket while riding his motorcycle.
6. Investigation team led by PINSP JIMALYN ABANCIA PAETE, DCOP, proceeded the
crime scene after he confirmation of the first responder, saw the declared dead body of the
victim and while conducting crime scene search they recovered 3 pcs of empty shell of caliber
45 pistol.
7. While crime scene search was on going, a call from PO2 Alex Santos was received by
investigation team about their arrest of the alleged suspect of the shooting incident whoa was
identified as Bryan Lopez y Sy, 34 years old, single and a resident of Brgy. Mayapa, Calamba
City, who voluntarily submitted himself to the arresting officer for an interview to be made at
Calamba CPS.
8. Evidences recovered the crime scene, cadaver of the victim for post mortem
examination and bullets for ballistics examination which results are to be presented in this
folder.
9. Affidavit of undersigned investigator of averred that he conducted the crime scene
investigation, interview witnesses, the alleged suspect, that he followed the operation procedure
of the PNP and he brought the suspect to the Prosecution Office for the conduct of Inquest
proceeding.

IV. DISCUSSION
10. During the interview of the alleged suspect conducted by SPO3 Jennica Mae P
Renomeron herein Calamba CPS, He requested to be questioned to the proper court and also
asked for assistance from Public Attorney’s Office through the whole duration of his trial.

V. CONCLUSION
11. After careful handling of the case, the undersigned investigator declared this as a
Cleared Case.

VI. RECOMMENDATION
12. WHEREFORE, our office recommends a strongly advice the witness of the shooting
incident to appear in court.
