AFFIDAVIT OF INVESTIGATOR

I, Jennica Mae P Renomeron, Of legal age, single, a resident of Brgy. Isidro, Calamba City, Laguna,
after having been duly sworn to an oath in accordance with law, hereby depose and say:

1. That, I am regular and active member of Philippine National Police with the rank of SPO3, presently
assigned and designated as the Chief, investigator of Calamba CPS, Calamba Laguna.

2. That, on 16, April 2016 at about 10:00 o’clock in the morning, I together with personnel of Calamba
CPS led by PINSP JIMALYN ABANCIA PAETE, DCOP, under the supervision and control of
PCINSP MA KRESTINE SALIGO, COP responded to a report alleged shooting incident by a
concern citizen, along Calumbang St. Brgy.Mayapa, Calamba City, wherein the victim was
identified as DARUIS MALABANAN y MERCADO, 37 years of age, a former security guard, a
resident of canlubang St. Brgy. Mayapa, Calamba City who was reported allegedly shot repeatedly
on his head by an unknown unman while walking beside Itlog ni Kuya Food House wearing a black
jacket that immediately flled towards an unknown direction using his unplated motorcycle. The
victim was declared DEAD ON THE SPOT by DR DIANELLE MAE R PALAÑA- Jose Rizal
Memorial District Hospital, Calamba City, Laguna, recovered at scene of the crime were three (3)
pieces of empty shells for cal 45, and blue sling bag containing personal items of the victim.

3. That, as investigator, I conducted interview with the owner and other employee of Itlog ni Kuta Food
house about they have seen and if they recognized the suspect which they actively participated.

4. That I exerted my efforts together with the Calamba MPS personnel as we conducted the “OPLAN
SITA” among the area of Calamba City in order to gather information about the incident that led to
identification and apprehension of the suspect named Daruis Malabanan Y Mercado. The
perpetrator will be appropriately charged and prosecuted in the court with the warranted evidences
and testimonial statement of the witnesses to give justice to the victim of the incident.

5. That, I am executing this Affidavit freely and voluntarily to attest to the truth of the foregoing facts
and to support this affidavit to this case.
